| Abstract | Fiksne kombinacije lijekova (FKL) su dugi niz godina predmet rasprave po nizu pitanja:
učinkovitosti, prihvatljivosti, isplativosti i sigurnosti. Danas, možemo tvrditi kako je upotreba
FKL u kliničkoj praksi postala neizostavna. Naime, u liječenju hipertenzije FKL zauzimaju
najveći dio tržišta lijekova kao i u još nekoliko polja medicine. Danas je velik broj znanstvenih
dokaza koji govore u prilog FKL u smislu poboljšanja suradljivosti pacijenata što je posebno
važno kod terapije kroničnih bolesti. Naime, kronične bolesti zahtijevaju od pacijenta striktno
pridržavanje terapijskog režima. Upravo je povećanje suradljivosti pacijenata jedna od glavnih
prednosti FKL uz ostale poput pojave sinergističke učinkovitosti, potencijalno olakšane
distribucije i smanjenih troškova te upotrebe u sekundarnoj prevenciji kroničnih bolesti. S druge
strane, postoje nedostatci terapijske strategije koja upotrebljava FKL. Naime, regulatorni uvjeti
pod kojima se nove FKL stavljaju u promet lijekova se značajno razlikuje između država što
proizvođačima znatno otežava povezivanje i održavanje opskrbnih lanaca. Također,
nemogućnost postepenog doziranja FKL je vrlo problematična iz razloga što temelji moderne
farmakologije počivaju na individualiziranom pristupu pacijentu. Ipak, kada se u obzir uzmu
različite potrebe s obzirom na raznolikost zdravstvenih sustava, FKL definitivno pronalaze svoje
mjesto u državama srednjeg i niskog prihodovnog razreda s izrazito velikim populacijama poput
Kine, Indije i Irana. Upravo je dostupnost i smanjenje proizvodnih troškova glavni adut FKL u
takvim vrstama zdravstvenih sustava jer povećavaju dostupnost zdravstvene skrbi uz smanjenje
troškova. Što se tiče razvijenih zemalja, iako se već nalaze u upotrebi, potrebno je daljnje
istraživanje kako bi se sa sigurnošću zaključilo kolike su točno prednosti FKL u smislu ishoda
liječenja, troškova i prihvatljivosti među liječnicima i pacijentima. |
| Abstract (english) | Fixed dose combinations (FDC) have been the subject of debate for many years on several
issues: efficacy, acceptability, cost-effectiveness, and safety. Today, we can argue that the use of
FDC in clinical practice has become indispensable. Namely, in the treatment of hypertension,
FDC occupies the largest part of the drug market as well as in several other fields of medicine.
Today, there is a variety of scientific evidence in favor of FDC in terms of improving patient
compliance, which is especially important in the treatment of chronic diseases. Namely, chronic
diseases require the patient to strictly adhere to the therapeutic regimen. Increasing patient
compliance is one of the main advantages of FDC, among others, such as the possibilty of
synergistic efficacy, potentially facilitated distribution and reduced costs, and use in the
secondary prevention of chronic diseases. On the other hand, there are shortcomings in the
therapeutic strategy that uses FDC. Namely, the regulatory conditions under which new FDC are
placed on the market of medicines differ significantly between countries, which makes it much
more difficult for manufacturers to connect and maintain supply chains. Also, the impossibility
of gradual dosage of FDC is very problematic because the foundations of modern pharmacology
are based on an individualized approach to the patient. Yet, when different needs are taken into
account given the diversity of health systems, FDC find their place in middle and low-income
countries with extremely large populations like China, India, and Iran. It is precisely the
availability and reduction of production costs that are the main trump card of FDC in such types
of health systems because they increase the availability of health care while simultenously
reducing costs. As far as developed countries are concerned, although they are already in use,
further research is needed to conclude with certainty exactly what the benefits of FDC are in
terms of treatment outcomes, costs, and acceptability among physicians and patients. |
