Abstract
Alzheimerova je bolest neurodegenerativna bolest rastuće prevalencije bez učinkovite terapije i nerazjašnjene patogeneze. Postoje dva pogleda na etiologiju i patogenezu, a to su amiloidna i mitohondrijska kaskadna hipoteza. Hipoenergoza i poremećaji metabolizma rani su nalaz i javljaju se godinama prije kliničkog razvitka bolesti što smješta mitohondrije u središte patogeneze kod obiju hipoteza. U suštini, narušena je funkcija metaboličkih enzima i oksidativne fosforilacije uz izmjenu strukture i ekspresije genoma te pogoršanje oksidativnog stresa. Navedeni poremećaji nisu izolirani, već si međusobno pojačavaju negativan učinak na funkciju mitohondrija i u konačnici neurona. Rastući je broj otkrivenih disfunkcija koja se javljaju u mitohondrijima dok nam fiziološka uloga mitohondrija i njihova važnost za normalno funkcioniranje stanica postaje jasnija. Otkrića normalnih i abnormalnih procesa u kojima mitohondriji sudjeluju međusobno se nadopunjuju i pružaju nam cjelovitiji uvid u problematiku te nam otvaraju nove mogućnosti intervencije i dijagnostike.
Abstract (english)
Alzeheimer`s disease is chronic, progressive neurodegenerative disease with increasing prevalence and without an effective treatment. Amyloid cascade hypothesis and mitochondrial cascade hypothesis are trying to explain the etiology and pathogenesis of the disease. Since it was found that hypoenergosis with accompanying hypometabolism are earliest findings which precede onset of clinically verifiable disease by years, dysfunctional mitochondria are thought to be key factors fueling the manifestation and progression of the disease. In a nutshell, activity of bioenergetic machinery is diminished and oxidative stress is increased in addition to structural changes of mtDNA and downregulated expression of its genes. All of these changes have negative impacts on each other while worsening the dysfuntion of mitochondria and neurons. In addition to our better understanding of the underlying dysfuntion, physiological functions are also being ellucidated. Together, they give us hope that new and more effective treatments and diagnostic tests are going to be developed in the near future.
