Abstract Ankilozantni spondilitis je česta vrsta spondiloartropatije. To je kronična, upalna, autoimuna reumatska bolest koja glavninom zahvada zglobove kralježnice, uzrokujudi kronične bolove, strukturne i funkcionalne poteškode kao što su smanjena pokretljivost i deformacija aksijalnog skeleta. ... More gubitka radne sposobnosti. Nefarmakološka terapija uključuje fizikalnu terapiju kao temeljni dio liječenja ankilozantnog spondilitisa. Hidroterapija ili terapija vodom, terapija bazenom, balneoterapija, jedna je od vodedih, široko rasprostranjenih metoda fizikalne terapije. Korištenje vode u različitim oblicima i različite temperature može proizvesti različiti učinak na organske sustave. Istraživanja o učinku hidroterapije u bolesnika s ankilozantnim spondilitisom pokazuju pozitivan učinak takve vrste liječenja na indekse aktivnosti za ankilozantni spondilitis, poglavito na smanjenje boli, povedanje pokretljivosti, poboljšanje funkcije i kvalitete života. Bolji povoljni učinak na tegobe bolesnika s ankilozantnim spondilitisom pokazuje istovremeno korištenje hidroterapije s farmakološkim liječenjem ili drugim oblicima nefarmakološkog liječenja u usporedbi sa provođenjem samo hidroterapije ili jednog oblika drugih vrsta terapije. Iako se bilježi napredak u količini i kvaliteti istraživanja na ovu temu, još uvijek ostaju otvorena pitanja vezana za kvalitetu kliničkih studija i znatnu varijabilnost metodologije. Ističu se potreba daljnjeg razvoja i usklađivanja rehabilitacijskih protokola i problemi uslijed pretjerane uporabe kvantitativnih metoda vrednovanja, kao prepreka primjene dosadašnjih rezultata u kliničkoj praksi, prema medicini utemeljenoj na dokazima (Evidence Based Medicine- EBM). Abstract (english) Ankylosing spondylitis is a common type of spondylorthropathy. It is a chronic, inflammatory, autoimmune rheumatic disease that mainly affects spine joints, causing severe chronic pain, structural and functional impairments, such as reduced mobility and axial deformity. It can reduce the quality of life and increase the risk of disability and mortality. It also causes direct and indirect economic losses due to health expenses and as a result of workforce loss. ... More include physical one as a part of the mainstreams of treatment of ankylosing spondylitis. Hydrotherapy, also called as water therapy, aquatic therapy, pool therapy or balneotherapy is one of the basic methods of treatment widely use as a part of physical therapy. Use of water in various forms and in various temperatures can produce different effects on different system ofthe body. The studies concerning hidrotherapy in patients with ankylosing spondylitis show a positive effect on ankylosing spondylitis indexes, especiallyon pain, mobility, function and quality of life. The simultaenous use of hydrotherapy with pharmacological treatment or other forms of non-pharmacological teratment has better effect compared to the implementation of only hidrotherapy or other types of therapy. Although there has been progess in the quantity and quality of researches, there are still open questions related to the quality of clinical studies and considerable variability in methodology. The need for further development and harmonization of rehabilitation protocols are highlighted. Problems due to excessive use of quantitive evaluation methods represent an obstacle to the application of current results in clinical practice, according to Evidence Based Medicine (EBM). Less
