Abstract Psorijatični artritis (PsA) je seronegativni artritis koji se često javlja u sklopu kožne psorijaze. Bolest zahvaća periferne i aksijalne zglobove, na kralježnici se javlja ankiloza, a u perifernim zglobovima vidljiv je erozivni artritis. Zbog sličnosti koje dijeli s reumatoidnim artritisom (RA), PsA nije oduvijek bio prihvaćen kao zasebna bolest. Ipak, otkriće reumatoidnog faktora (RF) označilo je prekretnicu te je PsA postao zaseban entitet, U prilog tomu su išla nova saznanja o patogenezi bolesti te otkrića o ulozi gena i okolišnih čimbenika u njenom nastanku. Nove spoznaje o bolesti s vremenom su dovele do otkrića brojnih dijagnostičkih i metoda liječenja PsA. Razvijeni su i brojni kriteriji koji pomažu pri otkrivanju bolesti i evaluaciji njene težine te smjernice koje pomažu pri odabiru ispravne terapije. U terapiji PsA danas se koriste brojna farmakološka sredstva poput sintetičkih i bioloških DMARD-ova uz pomoću kojih je moguće postići remisiju bolesti. Unatoč tomu, tjelesna aktivnost ostaje nezamjenjiva terapijska opcija u borbi sa psorijatičnim artritisom. Ciljeve u liječenju PsA predstvaljaju: smanjenje boli i ublažavanje simpotoma bolesti, prevencija deformiteta, očuvanje opsega pokreta, jačanje mišićne snage i čvrstoće kostiju te poboljšanje kondicije i kvalitete života. U ostvarenju svih tih ciljeva, uvleike pomaže tjelesna aktivnost. Ona ima pozitivan utjecaj na samu bolest, ali i na komorbiditete koji su vrlo često vezani uz nju, prvenstveno na one vezane uz kardiovskularni sustav. Zbog toga je nužno ukomponirati tjelesnu aktivnost u liječenje bolesnika sa PsA te ih educirati o njenoj važnosti.
Abstract (english) Psoriatic arthritis (PsA) is seronegative arthritis that often occurs as part of
cutaneous psoriasis. The disease affects the peripheral and axial joints, ankylosis
occurs in the spine, and erosive arthritis can be seen in the peripheral joints. Because
of the similarities it shares with rheumatoid arthritis (RA), PsA has not always been
accepted as a separate disease. However, the discovery of rheumatoid factor (RF)
marked a turning point and PsA became a separate entity. This was supported by new
knowledge about the pathogenesis of the disease and discoveries about the role of
genes and environmental factors in its development. New insights into the disease over
time have led to the discovery of a number of diagnostic and treatment methods for
PsA. Numerous criteria have been developed to help detect the disease and evaluate
its severity, as well as guidelines to help choose the right therapy. Today, a number of
pharmacological agents are used in the treatment of PsA, such as synthetic and
biological DMARDs, with the help of which it is possible to achieve remission of the
disease. Nevertheless, physical activity remains an indispensable therapeutic option
in the fight against psoriatic arthritis. The goals in the treatment of PsA are to reduce
pain and alleviate the symptoms of the disease, prevent deformities, maintain range of
motion, improve muscle and bone strength as well as fitness and quality of life. In
achieving all of these goals, physical activity helps greatly. It has a positive effect on
the disease itself, but also on the comorbidities that are very often associated with it,
primarily those related to the cardiovascular system. Therefore, it is necessary to
incorporate physical activity into the treatment of patients with PsA and educate them
about its importance.
