Kidney transplantation is the treatment of choice for patients with end-stage renal disease. Immunosuppressive therapy is required in the post-transplant period. Immunosuppressants prevent graft rejection, but also make the patient susceptible to developing infections. Infections are the second most common cause of death in renal transplant recipients after cardiovascular diseases. Despite prophylaxis, which significantly reduces the incidence of certain infections, they remain a significant cause of complications and can significantly affect the outcome of end-stage renal disease treatment. Fungal infections are the less common post-transplant complications, but have a high morbidity and mortality rate. They usually cause infections in immunocompromised patients such as renal transplant recipients, in whom they may present with an atypical clinical presentation. For this reason, they are challenging to diagnose and treat. Due to immunosuppressive therapy, it is necessary to pay special attention to the type of antifungal drugs and the dose used to avoid developing side effects and drug interactions. This paper will present the most common causes of fungal infections in renal transplant recipients, their clinical presentation, diagnosis and treatments.
