Abstract Napredak tehnologije, intenzivnog liječenja i njege bolesnika doveo je do nove kategorije
bolesnika, čije vitalne funkcije ovise o strojnoj potpori. Sve veći broj bolesnika koji
zahtijevaju dugotrajnu strojnu ventilaciju donosi produljenje njihovog života, niza etičkih,
ekonomskih i medicinskih problema. Infekcijske komplikacije koje se mogu javiti tijekom
liječenja bolesnika na dugotrajnoj strojnoj ventilaciji predstavljaju vrlo važan problem u
suvremenoj medicini, osobito u području intenzivne medicine. Infekcije dodatno pogoršavaju
ionako narušeno zdravstveno stanje kritično oboljelih, pridonose nepovoljnom ishodu
liječenja te su često razlogom dugotrajne hospitalizacije u JIL-u i visokih troškova
zdravstvene skrbi. Unatoč napretku u modernoj medicini, smatra se da otprilike 5-10%
bolesnika primljenih u bolnicu stekne infekciju povezanu sa zdravstvenom skrbi. Kako bismo
upozorili na taj problem proveli smo istraživanje prikupljanjem podataka iz medicinske
dokumentacije bolesnika u razdoblju od 1. siječnja 2016. godine do 31. 12. 2017. godine a
hospitalizirani su u JIL-u Specijalne bolnice za plućne bolesti na dugotrajnoj strojnoj
ventilaciji, koju smo definirali kao onu što u kontinuitetu traje mjesec dana i duže. Cilj ovog
retrospektivnog istraživanja bio je utvrditi učestalost i epidemiološka obilježja infekcija kod
bolesnika na dugotrajnoj strojnoj ventilaciji koji zahtijevaju intenzivnu skrb te ispitati moguće
rizične čimbenike odgovorne za pojavu infekcijskih komplikacija. Infekcije povezane sa
zdravstvenom skrbi su komplikacije u bolničkoj skrbi koje predstavljaju opasnost za
sigurnost bolesnika i opterećenje za zdravstveni sustav jer povećavaju mortalitet i morbiditet
i na taj način produljuju boravak bolesnika u bolnici i troškove njihova liječenja. Edukacija
zdravstvenih djelatnika i fokusirane intervencije, poznate kao propisani snop postupaka,
trebaju biti implementirane u bolnicama pošto su se pokazale kao učinkovite, jednostavne i
praktične u prevenciji infekcija povezanih sa zdravstvenom skrbi.
Abstract (english) Advances in technology, intensive care, and patient care have led to a new category of
patients whose vital functions depend on mechanical support. An increasing number of
patients requiring long-lasting mechanical ventilation bring a prolongation of their lives, a
series of ethical, economic and medical problems. Infectious complications that may occur
during the treatment of patients on long-term mechanical ventilation are a very important
issue in modern medicine, especially in the field of intensive medicine. Infections further
exacerbate the already compromised health condition of the critically ill, contribute to the
unfavourable outcome of treatment and are often the cause of long-term hospitalization in
JIL and high costs of health care. Despite (the) advances in modern medicine, it is
considered that in approximately 5-10% of hospital patients healthcare related infections
occur. To point out this problem, we conducted research by collecting data from medical
records of patients from January 1, 2016 to December 31, 2017, hospitalized at ICU in
Special Hospital for Pulmonary diseases on a long term mechanical ventilation , which we
defined as one that lasts for a month or longer. The aim of this retrospective study was to
determine the incidence and epidemiological features of long term mechanical ventilation
patients requiring intensive care and to examine the possible risk factors responsible for the
occurrence of infection complications. Infections associated with healthcare are
complications in hospital care that pose a risk to patient safety and are a burden on the
health system, as they increase mortality and morbidity, thereby prolonging the stay of
patients in hospital and increase the cost of their treatment. The education of healthcare
professionals and focused interventions, known as the prescribed set of procedures, should
be implemented in hospitals as they have proven to be effective, simple and practical in
preventing health-related infections.
