Abstract
Suvremena terapija papilarnog karcinoma štitnjače sastoji se od primarnog kirurškog liječenja, poslijeoperacijske radiojodne terapije i TSH supresijske hormonske terapije. Dok su kirurški protokoli za liječenje primarnog tumora i metastaza u prednju regiju vrata relativno dobro usuglašeni u stručnoj i znanstvenoj literaturi, pitanje indikacija i opsega kirurškog zahvata kod lateralnih metastaza na vratu (regije II.-V.) vrlo je proturječno te se u literaturi može naći široka lepeza po opsegu resekcije vrlo različitih zahvata preporučenih za tu indikaciju. Poseban prijepor odnosi se na potrebu rutinske disekcije podregije vrata II.b. Iz tog razloga provedeno je prospektivno istraživanje na 53 ispitanika s papilarnim karcinomom štitnjače i dokazanim metastazama u lateralnim regijama vrata, a s ciljem utvrđivanja optimalnog opsega kirurškog zahvata za lateralne metastaze vrata, kao i uspostavljanja jasnih prijeoperacijskih kriterija o potrebi disekcije podregije II.b kod pojedinog bolesnika. Na temelju dobivenih rezultata može se zaključiti da je selektivna disekcija vrata regija II.-V. optimalna kirurška metoda liječenja metastaza papilarnog karcinoma štitnjače u lateralnim regijama vrata te da nije potrebna rutinska disekcija podregije II.b kod svakog bolesnika s tom dijagnozom. Najznačajniji prijeoperacijski (ili intraoperacijski) kriterij za uključivanje podregije II.b u disekciju je pozitivan nalaz u podregiji II.a.
Abstract (english)
Actual treatment of papillary thyroid carcinoma consists of primary surgical treatment, postoperative radioiodine therapy and TSH-suppressive hormonal therapy. While surgical protocols for the treatment of a primary tumor and central compartment metastases are relatively well agreed in professional and scientific literature, the indications and extent of surgery for lateral neck metastases (neck levels II-V) are still controversial and one can find a variety of procedures recommended for this indication. Particular controversy concerns the need for routine neck dissection of sublevel IIb. For these reasons, a prospective study was designed that included 53 patients with proven lateral neck metastases from papillary thyroid carcinoma, with the aim to determine the optimal surgical procedure for lateral neck metastases and to establish distinct individual criteria for the need of sublevel IIb dissection. According to the results of this study, selective neck dissection of levels IIV is the optimal surgical procedure in the treatment of papillary thyroid carcinoma metastases in lateral neck levels, and there is no need for routine dissection of sublevel IIb. The most important preoperative (or intraoperative) criterion for sublevel IIb dissection is positive finding in sublevel IIa.
