ocijeniti povezanost atopije u obitelji, te učinak faktora okoliša radi ocjene rizika za razvoj atopijskih bolesti. ----- Ispitanici i metode rada: Prva faza ispitivanja je provedena od siječnja do travnja 2005. godine među djecom školske dobi (3106 djece od 7 do 9
godina) u 27 osnovnih škola izabranih metodom slučajnog odabira. Podaci o prevalenciji simptoma atopijskih bolesti prikupljeni su
standardiziranim ISAAC pisanim upitnikom faze I. Druga faza ispitivanja je provedena tijekom školske godine 2007./08. U ispitivanje su uključena
djeca iz prve faze studije (sada u dobi od 10. do 12. godine života) te je izdvojeno 300 djece sa simptomima alergijskih bolesti i 300 zdrave djece
odabrane slučajnim odabirom. U obje skupine provedeno je ispitivanje korištenjem upitnika druge faze ISAAC studije o rizičnim čimbenicima. Podaci o
učestalosti infekcija i povišene tjelesne temperature (iznad 38,5 °C), učestalosti primjene antibiotika i paracetamola, podaci o vakcinaciji te
rezultata PPD testa su prikupljeni uvidom u medicinsku dokumentaciju primarne zdravstvene zaštite. Djeca sa simptomima atopijskih bolesti su
alergološki obrađena u Dječjoj pulmoalergološkoj ambulanti u Županijskoj bolnici Čakovec, a kod zdrave djece je od obrade učinjen kožni skin
prick test na inhalacijske alergene. ----- Rezultati: 22.9% djece je ikada imalo simptome atopijskih bolesti. Učestalost simptoma tijekom života je iznosila: piskanje 14.91%, simptomi
alergijskog rinitisa 8.88%, te simptomi atopijskog dermatitisa 10.66%. Ispitani su i simptomi atopijskih bolesti prisutni unatrag 12
mjeseci: piskanje 6.92%, simptomi alergijskog rinitisa 8.40%, simptomi atopijskog dermatitisa 5.76%. Najvažniji rizični čimbenici za razvoj
atopijskih bolesti su pozitivna obiteljska anamneza i senzibilizacija na inhalacijske alergene. Djeca sa simptomima atopijskih bolesti su
češće izložena pasivnom pušenju, grijanju na ugljen, virusnim respiratornim infekcijama, učestalijoj upotrebi paracetamola pri nižim vrijednostima
povišene tjelesne temperature i antibiotika, češće su pohađala kolektiv te su kraće dojena. Djeca bez simptoma atopijskih bolesti su češće
izložena kućnim ljubimcima i domaćim životinjama, češće spavaju na jastuku od perja i na madracu starijem od 3 godine i posjeduju
tepisone, češće se procijepljena za hepatitis virus B te imaju pozitivan PPD test. Multivarijantnom analizom najvažnijim rizičnim čimbenikom
za razvoj simptoma atopijskih bolesti je pozitivna obiteljska anamneza. ----- Zaključak: rezultati pokazuju da je Međimurska županija područje s umjerenim stupnjem prevalencije simptoma alergijskih bolesti među djecom
školske dobi. Od rizičnih čimbenika za razvoj atopijskih bolesti najvažniji su pozitivna obiteljska anamneza i senzibilizacija na
inhalacijske alergene.
Abstract (english) Aim of the study: To estimate the prevalence of asthma, allergic rhinitis and atopic eczema symptoms in school children in the Medjimurje County
and to estimate the influence of positive family atopy and the environmental risk factors on development of atopy diseases. ----- Subjects and methods: First phase of the study was undertaken between January and April 2005 among the 3106 7-9 year-old school children in 27
randomly selected elementary schools. Data were collected using standardized ISAAC written questionnaire Phase I. Second phase of the study was
undertaken on 2007/08 school year among randomly selected children from the first phase (300 children with and 300 children without symptoms of
atopy diseases, now aged 10-12). Data about family atopy and environmental risk factors were collected using standardized ISAAC written
questionnaire Phase II. Data about respiratory infections, fever, use of antibiotics and paracetamol, vaccination and PPD results were collected
from medical documentation. Children with symptoms of atopy diseases were examined in Children's Pulmo Allergological Ambulance in County
Hospital Čakovec, and children without symptoms of atopy diseases had Skin prick test preformed. ----- Results: 22.9% of the children had symptoms of atopy diseases at some time in their life. Estimated lifetime (ever) prevalence rates of symptoms
were: wheezing 14.91%, allergic rhinitis symptoms 8.88%, and atopic dermatitis symptoms 10.66%. Estimated 12-month prevalence rates
were: wheezing 6.92%, allergic rhinitis symptoms 8.40%, and atopic dermatitis symptoms 5.76%. The most important risk factors for development
of atopy diseases are positive family atopy and sensitization to inhalant allergens. Children with symptoms of atopy diseases are more often
exposed to secondhand smoke, coal heating, virus respiratory infections, more frequently use of paracetamol and antibiotics, kindergarten
and shorter breastfeeding. Children without symptoms of atopy diseases are more often exposed to pets and farming animals, feather
pillows, mattresses older than 3 years and own carpets, more vaccinated for Hepatitis virus B and have positive PPD test. In multivariate
analyses the most important risk factor is positive family atopy. ----- Conclusions: The results of this study show that Međimurje County is a region with moderate prevalence of atopic diseases among the pediatric
population. The most important risk factors for development of atopy diseases are positive family atopy and sensitization to inhalant allergens.
