Abstract Uvod: Prezentizam je pojava kad zaposlenici dođu na posao, ali iz različitih razloga ne rade
svoj posao na odgovarajući način. Razlog prezentizmu mogu biti i zdravstvene poteškoće
(prezentizam zbog bolesti). Suprotnost prezentizmu je apsentizam - pojava kad zbog tih
poteškoća zaposlenici izostaju s posla. Kultura bolesnikove sigurnosti rezultat je obrazaca
ponašanja i stavova prema zdravlju i sigurnosti u zdravstvenoj organizaciji.
Cilj: Ispitati učestalost prezentizma i apsentizma u bolnicama; njihov odnos s kulturom
bolesnikove sigurnosti. Temeljem rezultata dati prijedlog mogućih mjera prevencije
prezentizma i apsentizma u bolnici.
Ispitanici: Liječnici i medicinske sestare; 399 iz kliničkog bolničkog centra i 196 iz opće
županijske bolnice.
Metode: Presječno istraživanje provedeno je upitnicima Stanfordska ljestvica za mjerenje
prezentizma (SPS-6), Svjetske zdravstvene organizacije za mjerenje zdravlja i radne
učinkovitosti (WHO HPQ), Bolničko istraživanje o kulturi bolesnikove sigurnosti (HSPOSC).
Distribucije kvanitativnih varijabli su opisane konvencionalnim mjerama deskriptivne
statistike, analizirane t-testom i ANOVA analizom, a kvalitativnih varijabli χ2 testom i
Fisherovim egzaktnim testom. Redukcije dimenzionalnosti prostora kulture bolesnikove
sigurnosti učinjena je faktorska analiza po komponentnim modelom primjenom Keiserov
kriterija uz rotaciju Varimax. Odnos prezentizma i apsentizma s kulturom bolesnikove
sigurnosti, analiziran je linearnom regresijskom i diskriminacijskom analizom.
Rezultati: Složenost i uzajamnost odnosa prezentizma i apsentizma odrazila se u naizgled
proturječnim rezultatima proizašlim iz heterogenosti važnosti pojedinih prediktora u procjeni
kriterija na različitim razinama događaja, odnosno analize (individualna razina, odjel,
bolnica). Tek analiza po pojedinim bolnicama, odjelima i zanimanjima daje podatke koje je
moguće interpretirati.
Zaključak: Prezentizam, apsentizam i kultura bolesnikove sigurnosti, dio su kulture neke
zajednice, u konkretnom slučaju odjela i bolnice. Pojave oba promatrana oblika prezentizma
(rada sa smanjenim učinkom zbog bolesti ili iz drugih razloga) kao i apsentizma, učestalije su
grupirane na pojedinim odjelima i u pojedinim zanimanjima.
Abstract (english) Introduction: Presenteeism is the situation when employees show up, but fail to adequately
work for various reasons. Reasons can be health-related (sickness presenteeim). Opposite,
absenteeism is when employees are absent from work. Patient safety culture includes attitudes
and behavioural patterns towards health and safety in a healthcare organisation.
Aim: To investigate the prevalence of presenteeism and absenteeism in hospitals; their
relationship with patient safety culture. Based on the results give possible presenteeism and
absenteeism preventive measures.
Prticipants: Medical doctors and nurses; 399 of from the clinical hospital center and 196
from county general hospital.
Methods: The cross-sectional study made by questionnaires: Stanford Presenteeism Scale
(SPS-6), WHO Health and Work Performance Questionnaire (WHO HPQ), Hospital Survey
on Patient Safety Culture (HSPOSC) and Perceived Stress Scale (PSS-10). Distributions were
described by conventional measures of descriptive statistics and analyzed using t-test and
ANOVA analysis. Distributions of qualitative data were analysed using the χ2 test and the
Fisher exact test. Data were examined using explorative factor analysis (PCA with Varimax
rotation), to determine dimensionality reduction of patient safety culture space. Linear
regression and discriminant analysis were used to asses the relationship between
presenteeism, absenteeism and patient safety culture.
Results. The complexity and reciprocity of relations of presenteeism and absenteeism
reflected in the seemingly contradictory results, arising from the heterogeneity of the
importance of certain predictors in assessment criteria at different levels of events and
analysis (individual level, department, hospital). Only analysis by individual hospitals,
departments and occupations provides data that can be interpreted.
Conclusion: Presenteeism, absenteeism and patient safety cutlture are a part of the culture in
the specific case of departments and hospitals. Presenteeism and absenteeism, are frequently
grouped in individual departments and in some professions, which supports their property as a
form of behavior.
