Abstract
Uvod: Periferna arterijska bolest (PAB) jedna je od najčešćih manifestacija ateroskleroze. Iako su istraživanja pokazala da karotidna bolest (KB) i PAB dijele zajedničke faktore rizika, prognostičko značenje KB u bolesnika sa simptomatskom PAB, te njihova korelacija s upalnim biomarkerima, nedovoljno je istražena. Cilj: Primarni cilj istraživanja bio je odrediti učestalost i prognostičko značenje KB u bolesnika sa simptomatskom PAB te povezanost KB s upalnim bimarkerima. Sekundarni ciljevi bili su istražiti prognostičko značenje KB u različitim subpopulacijama bolesnika sa simptomatskom PAB uključujući bolesnike sa šećernom bolešću, koronarnom bolešću, kritičnom ishemijom ekstremiteta (CLI) i kroničnom bubrežnom bolešću (KBB). Ispitanici i metode: U istraživanje je uključeno 454 bolesnika sa simptomatskom PAB (Rutherford stadij 2 - 6). Tijekom hospitalizacije zabilježeni osnovni klinički i demografski podaci, podaci o KV faktorima rizika, te laboratorijski podaci. Učinjen je UZV pregled karotidnih arterija. Svi bolesnici praćeni su do pojave prvog velikog neželjenog KV događaja (MACE) koji je uključivao jedan od sljedećih ishoda: nefatalni infarkt miokarda (IM), moždani udar te smrt KV uzroka. Rezultati: Među bolesnicima sa simptomatskom PAB, od kojih je 40% imalo CLI, najčešći faktori bili su hipertenzija i dislipidemija. Karotidna bolest bila je prisutna u 39% bolesnika, a PVB u 58% bolesnika. Među bolesnicima s KB, tri četvrtine je imalo asimptomatsku bolest. Većina bolesnika s KB imali su umjerenu karotidnu stenozu, a najzastupljenija vrsta karotidnog plaka bili kalcificirani i mješoviti. Bolesnici s KB bili su stariji, češće su imali KBB i anemiju te povišene upalne biomarkere. U logističkoj regresijskoj analizi dob i C-reaktivni protein (CRP) bili su nezavisno povezani s KB. Tijekom vremena praćenja od 49 mjeseci, 43% bolesnika doživjelo je MACE, a 40% smrtni ishod. Dob, šećerna bolest, CLI, koronarna bolest i CRP bili su nezavisno povezani s MACE-om. Karotidna bolest bila je povezana s MACE-om u univarijatnoj analizi, ali ne i u multivarijatnoj analizi. Analizirajući faktore povezane s ukupnim mortalitetom, dob, šećerna bolest, CLI, koronarna bolest, CRP i anemija bili su nezavisni pretkazatelji ukupnog mortaliteta. Zaključak: Bolesnici sa simptomatskom PAB imaju često prisutnu KB koja je u većine bolesnika asimptomatska. Pridružena KB povezana je s kroničnom sistemskom upalom, a navedeni bolesnici često imaju pridružene komorbiditete poput KBB i anemije. Učestalost MACE-a i ukupnog mortaliteta među bolesnicima sa simptomatskom PAB je značajna te stoga traganje za pridruženom KB predstavlja dodatni alat u stratifikaciji rizika.
Abstract (english)
Introduction: Peripheral artery disease (PAD) is one of the most common manifestations of atherosclerosis. Although PAD and carotid disease (CD) share common risk factors, the prognostic impact of CD in patients with PAD, is poorly studied. Aim: We investigated prevalence and prognostic significance of CD in PAD patients, and its correlation with inflammatory biomarkers. Methods: Study included 454 consecutive patients who were hospitalized for symptomatic PAD. Patients were followed up for the occurrence of the first major adverse cardiovascular event (MACE), defined as composite endpoint of nonfatal myocardial infarction, stroke, and cardiovascular death. Results: Carotid disease had 39% of patients. Age and CRP were independently associated with CD. During follow up of 49 months, 43% of patients experienced MACE, and 40% of patients died. Age, diabetes, critical limb ischemia (CLI), coronary artery disease (CAD), and C-reactive protein (CRP) independently predicted MACE. Carotid disease was associated with MACE in the univariate analysis, but not in the multivariate analysis. Age, diabetes, CLI, CAD, CRP, and anemia independently predicted all-cause mortality. Conclusion: Patients with PAD have often concomitant CD which is associated with systemic inflammation. The occurrence of MACE and all-cause mortality among patients with PAD is significant, and hence looking for CD, may be useful risk stratification tool.
