Abstract
Humani bokavirus (HBoV) je virus iz porodice parvovirusa otkriven 2005. godine iz uzoraka respiratornog sustava djece sa akutnom infekcijom dišnog puta. Identificirana su četiri tipa HBoV-a; HBoV1 je primarno detektiran u dišnom sustavu, a HBoV2-4 u probavnom sustavu. Istraživanja pokazuju da je HBoV1 patogen koji uglavnom uzrokuje bolesti dišnog sustava dječje dobi dok uzročno-posljedična veza između detekcije HBoV2-4 i bolesti probavnog sustava još nije dokazana. Najčešći klinički entiteti bokavirusne infekcije u djece uključuju akutni bronhiolitis, pneumoniju, rinitis i akutni otitis media (AOM). Bolesti su uglavnom blagog tijeka i ne zahtijevaju hospitalizaciju, ali prijavljeni su slučajevi životno ugrožavajućih i fatalnih infekcija. Viremija tijekom akutne infekcije govori u prilog sistemskoj infekciji sa zahvaćanjem različitih organskih sustava. Poželjno je da virološka dijagnostika akutne bokavirusne infekcije uključuje kombinaciju molekularnih i seroloških metoda. Naime, karakteristika bokavirusne infekcije je produljeno izlučivanje virusa nakon akutne infekcije i česta kodetekcija/koinfekcija HBoV s ostalim respiratornim virusima što otežava interpretaciju pozitivnih nalaza. Rizični čimbenici za bokavirusnu infekciju težeg kliničkog tijeka su, osim dječje dobi, imunodeficijencija i postojeće kronične bolesti dišnog sustava. Zasad ne postoji ciljani lijek ili cjepivo za HBoV te osnovu liječenja infekcije čini podupiruća terapija. HBoV je važan uzročnik bolesti dišnog sustava stoga je infekciju potrebno pravovremeno dijagnosticirati i pratiti njen klinički tijek radi preveniranja mogućih komplikacija u visokorizičnoj populaciji.
Abstract (english)
Human bocavirus (HBoV) is a DNA virus belonging to the parvovirus family. It was discovered in 2005 in respiratory samples from children with acute respiratory tract infections. Four types of HBoV have been identified; HBoV1 is primarily detected in respiratory tract samples as opposed to HBoV2-4 which are primarily detected in the gastrointestinal tract. Various types of research show that HBoV1 is a true pathogen that mainly causes respiratory tract disease in children. The causal relationship between HBoV2-4 detection and gastrointestinal disease isn't proved yet. Frequent clinical features of bocaviral infection include acute bronchiolitis, pneumonia, rhinitis, and AOM. The disease course is often mild and similar to that caused by other respiratory viruses. There've been, however, cases of life-threatening and fatal infections. Viremia during acute infection suggests a systemic infection that can affect multiple organs. Laboratory diagnosis of acute bocaviral infection must include a combination of molecular and serological methods. Common findings in bocaviral infection include a high rate of detected co-infections and prolonged shedding of the virus which makes it hard to interpret positive results. Risk factors for severe bocaviral infection are young age, immunodeficiency, and existing chronic respiratory tract disease. So far there is no specific medicine and/or vaccine for bocavirus and the main therapeutic approach is supportive therapy. It is necessary to consider HBoV as a true pathogen so a prompt diagnosis can be made and complications prevented in a high-risk population.
