Abstract
Mehanička cirkulacijska potpora naziv je za skupinu uređaja koja se koristi kao potpora srcu u terapiji zatajivanja srca (i/ili pluća). Ovisno o namjeni i trajanju samog postupka, mehanička cirkulacijska potpora može biti kratkoročna ili dugoročna, koristiti se tijekom kardiokirurških intervencija ili do transplantacije srca. Mehanička cirkulacijska potpora ima široku primjenu. Osim terapije kroničnog srčanog zatajenja, mehanička cirkulacijska potpora poput ECMO se koristi kao most prema transplantaciji pluća i u stanjima reverzibilne respiracijske insuficijencije, u liječenju pacijenata u stanju hipotermije s tahiaritmijama, reanimaciji... Uređaji koji se koriste za kratkoročnu potporu su IABP, Impella, ECMO i VAD, dok su u dugoročnoj cirkulacijskoj potpori najprimjenjivaniji VAD i TAH. Intrakorporalni uređaji ugrađuju se kirurški implantacijom pumpi u prsište, a perkutani i ekstrakorporalni uređaji rade na principu kanilacije većih vena i arterija do samog srca odakle dreniraju krv. Uređaji pružaju potporu jednom ili oba ventrikula. U praksi je moguća kombinacija jedne ili više vrsti uređaja za srčanu i/ili respiratornu potporu.
Abstract (english)
Mechanical circulatory support (MCS) is a group of devices used for treatment of cardiac (and/or respiratory) failure. Depending of purpose and duration of therapy, we distinguish long-term and short-term MCS used during cardiac surgery interventions or as bridge to heart transplant.Mechanical circulatory support has wide range of uses. MCS like ECMO can be used as bridge to lung transplant, in cases of reversible respiratory insuficiency, as treatment of hypothermia with tachyarrytmias, during reanimation... Devices used for short term support are IABP, Impella, ECMO and VAD, whereas VAD and TAH are mostly used during long-term circulatory support. Intracorporeal devices are positioned by implantation of pumps pericardialy, while percutaneus and extracorporeal devices require cannulation of greater vessles, draining blood from the heart. In practise one or more type of devices can be combined for cardiac and/or respiratory support.
