Abstract Buergerova bolest, još poznata pod nazivom thromboangiitis obliterans (TAO), je progresivna ne-aterosklerotska okluzivna bolest malih i srednje velikih krvnih žila ekstremiteta. Raširenija je u određenim geografskim regijama, predominantno na Bliskom Istoku i Aziji. ... More nastanak i razvoj bolesti. Klinička slika TAO karakterizirana je pojavom intermitentnih klaudikacija, ishemijskih ulkusa i gangrene koja prvenstveno zahvaća ruke i stopala. Rjeđe može uključivati velike arterije ili se može prezentirati multisistemskom zahvaćenošću i općim simptomima. Te su manifestacije rezultat tromboze i upalne okluzije zahvaćenih krvnih žila, što dovodi do smanjenog protoka krvi i ugroze tkiva. Trenutno se dijagnoza TAO oslanja na isključivanje drugih vrsta vaskulitisa i ateroskleroze pošto još nije otkriven specifičan marker za TAO. Dijagnostički proces obično uključuje kombinaciju kliničke procjene, laboratorijskih i slikovnih pretraga te vaskularna ispitivanja. Potpun prestanak pušenja je najučinkovitiji pristup u liječenju bolesti. Lijekovima se nastoji poboljšati protok krvi i bolje kontrolirati simptomi, a u teškim slučajevima mogu biti potrebni i invazivniji postupci. Less
Abstract (english) Buerger disease, also known as thromboangiitis obliterans (TAO), is a progressive non-atherosclerotic occlusive disease of small and medium-sized blood vessels in the extremities. It is more prevalent in certain geographical regions, predominantly in the Middle East and Asia. It primarily affects young smokers, mostly men, but nowadays an increasing number of cases are being observed among the female population. ... More the main risk factor for the development and progression of the disease. The clinical presentation of TAO is characterized by the occurrence of intermittent claudication, ischemic ulcers, and gangrene primarily affecting the hands and feet. Less commonly, it can involve large arteries or present with multisystem involvement and general symptoms. These manifestations are a result of thrombosis and inflammatory occlusion of the affected blood vessels, leading to reduced blood flow and tissue damage. Currently, the diagnosis of TAO relies on excluding other types of vasculitis and atherosclerosis since no specific marker for TAO has yet been discovered. The diagnostic process typically involves a combination of clinical assessment, laboratory and imaging tests, and vascular studies. Complete smoking cessation is the most effective approach in treating the disease. Medications aim to improve blood flow and have better symptom control, and in severe cases invasive procedures may be necessary. Less
