Abstract
Spondiloartritisi su grupa kroničnih upalnih reumatskih bolesti koje dijele zajedničke karakteristike. Pod tim terminom ubrajaju se radiološki i neradiološki aksijalni spondiloartritis, psorijatični artritis, reaktivni artritis i enteropatski artritis. Zbog negativnog serološkog nalaza reumatoidnog faktora nazivaju se još i seronegativne spondiloartropatije. Etiopatogeneza nije u potpunosti razjašnjena, ali smatra se da kombinacija genetskih i okolišnih čimbenika pridonose razvoju bolesti. Bolesti iz ove skupine pokazuju visoku pozitivnu korelaciju s HLA-B27. Zbog svoje kompleksnosti, dijagnoza može biti odgođena. Liječenjem se, između ostalog, želi postići kontrola upale kao i poboljšanje kvalitete života. Početno liječenje sastoji se od kombinacije fizikalne terapije i konvencionalne medikamentne terapije, uz edukaciju pacijenata. Razvojem tehnologije i boljim upoznavanjem patofiziologije upalnih bolesti omogućene su nove terapijske opcije koje uključuju inhibitore TNF-ɑ, IL-17, IL-12/23, IL-23 i male molekule. Biološka terapija i male molekule uvode se u terapiju u slučaju iscrpljenja klasičnih metoda liječenja.
Abstract (english)
Spondyloarthritides represent a group of chronic inflammatory rheumatic diseases that share common characteristics. This term includes radiological and non-radiological axial spondyloarthritis, psoriatic arthritis, reactive arthritis and enteropathic arthritis. Because of the negative serological finding of rheumatoid factor, they are also called seronegative spondyloarthropathies. The etiopathogenesis has not been fully understood, but it is believed that a combination of genetic and environmental factors contribute to the development of the disease. Diseases from this group show a high positive correlation with HLA-B27. Due to its complexity, the diagnosis can be delayed. Treatment aims, among other things, to control inflammation and improve quality of life. Initial treatment consists of a combination of physical therapy and conventional drug therapy, along with patient education. The development of technology and a better understanding of the pathophysiology of inflammatory diseases have enabled new therapeutic options, which include inhibitors of TNF-ɑ, IL-17, IL-12/23, IL-23 and small molecules. Biological therapy and small molecules are introduced into therapy in case of exhaustion of classical treatment methods.
