Abstract
Multipla skleroza (MS) kronična je demijelinizacijska bolest središnjeg živčanog sustava (SŽS) posredovana imunosnim čimbenicima. Najčešći je uzrok invaliditeta među mlađim odraslim osobama. MS nastaje na temelju genetske osnove uz utjecaj čimbenika okoliša, kao što su infekcija EBV-om, pušenje, manjak vitamina D i pretilost. Bolest započinje kao klinički izolirani sindrom (CIS), koji se tipično prezentira kao optički neuritis ili parcijalni transverzalni mijelitis. U većine oboljelih MS ima relapsno-remitirajući tijek (RRMS) koji traje 10 do 15 godina, kada MS postaje sekundarno progresivna bolest (SPMS). Manji broj bolesnika od početka ima primarno progresivan oblik bolesti (PPMS). Dijagnoza se postavlja na temelju kliničke slike i nalaza magnetske rezonance (MR) demijelinizacijskih lezija. Liječenje MS-a doživjelo je ogroman napredak u zadnja tri desetljeća zahvaljujući razvoju mnogih lijekova koji modificiraju tijek bolesti (DMT). Široki spektar motornih, senzornih, autonomnih i kognitivnih simptoma ometa svakodnevne aktivnosti oboljelih od MS-a i vidno narušava kvalitetu života. Simptome MS-a bitno je na vrijeme prepoznati i pravovremeno liječiti. Takvo liječenje je kompleksno te zahtijeva multidisciplinaran i multimodalan pristup.
Abstract (english)
Multiple sclerosis is a chronic, predominantly immune-mediated disease of the central nervous system, and the most common cause of neurological disability in young adults. The underlying cause of MS is a complex interaction between genetic and environmental factors, such as EBV infection, smoking, low serum levels of vitamin D and obesity. MS tipically starts as clinically isolated syndrome (CIS) that often presents as optic neuritis or parcial transverse myelitis. Most of the MS patients have a relaps-remitting disease course (RRMS), lasting 10 to 15 years until it becomes secondary progressive disease (SPMS). Minority of patients have a primary progressive onset (PPMS). Diagnosis is based on clinical presentation and MRI appearences of demyeliniating lesions. The treatment of MS has been advanced in the last three decades with development of disease modyfing therapies (DMT). A wide range of motor, sensory, autonomic and cognitive syptoms has a disturbing impact on daily activities and quality of life of MS patients. Early recognition of syptoms is very important to start the treatment in time. That management is complex and requires a multidisciplinary and multimodal approach.
