Abstract
Urolitijaza je postojanje kamenaca u mokraćnom sustavu i to je treća najčešća patologija urinarnog sustava. Većina kamenaca je u kanalnom sustavu bubrega, a manji broj u mokraćovodu i mokraćnom mjehuru. Izvantjelesna litotripsija udarnim valovima (ESWL) neinvazivna je metoda kojom se pomoću snage udarnih valova kamenci lome u manje dijelove, pa ovako nastali fragmenti izlaze iz tijela mokrenjem. Glavna indikacija za ovu metodu su kamenci veličine do 2 cm u promjeru. ESWL može biti vrlo učinkovita metoda, međutim postupak se kod nekih bolesnika treba ponavljati da bi se postiglo potpuno uklanjanje kamenaca. Sama metoda je sigurna, a komplikacije najčešće nisu značajne i traju kratko te uglavnom nastaju kao posljedica prolaska fragmentiranih kamenaca kroz mokraćni sustav. Kontraindikacije za ESWL su trudnoća, koagulopatija ili uzimanje antiagregacijske terapije, aneurizme aorte, nekontrolirana hipertenzija i infekcije mokraćnog sustava. Dosadašnja istraživanja nisu pokazala štetno djelovanje ESWL-a na bubrežnu funkciju. Kod kamenaca koji nisu pogodni za ovu metodu, odnosno kod bolesnika koji imaju kontraindikacije za ESWL mogu se koristiti druge metode liječenja kamenaca kao što su endoskopski, ali i otvoreni operativni zahvati. ESWL se koristi i u liječenju urolitijaze u djece.
Abstract (english)
Urolithiasis refers to the presence of calculi anywhere along the course of the urinary tract and it's the third most common urinary tract disease. Stones mainly occur in the kidneys, less frequently in the ureter and the bladder. Extracorporeal Shock Wave Lithotripsy (ESWL) is a noninvasive procedure that uses high-energy shock waves to break down the kidney stones into fragments small enough to eliminate from the body along with the urine. ESWL is considered a first-line treatment for renal calculi under 2.0 cm. ESWL is a highly effective method but sometimes stone fragments of various sizes remain and a repeat procedure is needed. The procedure is safe, and complications are mostly short-termed and mainly connected to the formation and passage of fragments down the urinary tract. Absolute contraindications to ESWL include pregnancy, coagulopathy or use of platelet aggregation inhibitors, aortic aneurysms, severe untreated hypertension, and untreated urinary tract infections. There is no firm evidence for long-term adverse effects on renal function. In patients with larger stones or in the presence of unfavourable factors, endourological procedures are prefered over ESWL. ESWL is an effective and safe modality to treat pediatric urolithiasis cases.
