Abstract
Teške infekcije SŽS-a u bolesnika zaraženih HIV-om imaju visok morbiditet i mortalitet čak i u eri primjene ART-a. Mogu biti uzrokovane oportunističkim patogenima, samim djelovanjem HIV-a ili nastati u sklopu sindroma imunološke inflamatorne rekonstitucije (engl. Immune reconstitution inflammatory syndrome, IRIS). U ovom istraživanju obuhvatili smo HIV-om zaražene bolesnike liječene zbog teških infekcija SŽS-a u Zavodu za intenzivnu medicinu i neuroinfektologiju Klinike za infektivne bolesti „Dr. Fran Mihaljević“ u Zagrebu, od 1.1.1996. do 31.12.2021. godine. Prikupljeni su opći demografski i epidemiološki podaci te značajke vezane uz HIV infekciju i intenzivno liječenje. Bolesnike smo podijelili u tri skupine, ovisno o prethodnoj primjeni ART-a, na novodijagnosticirane, bolesnike koji su adherentno primjenjivali ART i bolesnike koji su znali svoj HIV status, ali nisu adherentno uzimali terapiju. Dakle, promatrali smo vrstu infekcije, AIDS definirajuće bolesti te ishode liječenja za navedene skupine bolesnika. Od ukupno 98 HIV-om zaraženih osoba liječenih na Zavodu, 29 bolesnika (29.6%) liječeno je zbog infekcije SŽS-a. Među našim bolesnicima najviše je bilo novodijagnosticiranih osoba (20, 69.0%), odnosno kasnih prezentera koji su uglavnom obolijevali od oportunističkih infekcija i imali razne AIDS definirajuće bolesti. Ishodi kliničkog liječenja ovih bolesnika bili su nepovoljni i praćeni visokom smrtnosti (50.0%). Bolesnici zaraženi HIV-om koji su bili neadherentni u liječenju ART-om također su imali visoku smrtnost. Najbolji ishodi liječenja opaženi su kod bolesnika koji su adherentno koristili ART. Ti su bolesnici imali 100%-tno preživljenje i bolji oporavak funkcionalnog statusa tijekom praćenja, opisanog pomoću modified Rankin Scale (mRS).
Abstract (english)
Severe central nervous system infections in HIV-infected patients have high morbidity and mortality even in the era of ART. They can be caused by opportunistic pathogens, the action of HIV itself, or arise as part of the immune reconstitution inflammatory syndrome (IRIS). In this study, we included HIV-infected patients treated for severe infections of the CNS in the Department of Intensive Care Medicine and Neuroinfectology of the Clinic for Infectious Diseases "Dr. Fran Mihaljević" in Zagreb, from January 1, 1996. until 31/12/2021. General demographic and epidemiological data and characteristics related to HIV infection and intensive treatment were collected. We divided the patients into three groups, depending on the previous use of ART, into newly diagnosed patients, patients who adhered to ART and patients who knew their HIV status, but did not adhere to the therapy. Therefore, we assesed the type of infection, the AIDS-defining disease, and the treatment outcomes for the aforementioned groups of patients. Out of a total of 98 HIV-infected persons treated at the Institute, 29 patients (29.6%) were treated for CNS infection. Among our patients, the majority were newly diagnosed persons (20, 69.0%), i.e. late presenters who mostly suffered from opportunistic infections and suffered from various AIDS-defining diseases. The results of the clinical treatment of these patients were unfavorable and accompanied by high mortality (50.0%). HIV-infected patients who were nonadherent to ART treatment also had high mortality. The best treatment outcomes were observed in patients who adhered to ART. These patients had 100% survival and better recovery of functional status during follow-up, as described using the modified Rankin Scale (mRS).
