Abstract
Autoinflamatorne bolesti kostiju su rijetke i njihovo glavno obilježje je sterilna upala kostiju koja je praćena kožnom patologijom. Obuhvaćaju različite sindrome: sinovitis, akne, pustuloza, hiperostoza, osteitis (SAPHO sindrom), kronični rekurirajući multifokalni osteomijelitis (CRMO), Majeedov sindrom, kerubizam i druge. Sindromi zahvaćaju više organskih sustava te se često preklapaju s drugim bolestima zbog čega nerijetko ostanu neprepoznati. Rana dijagnostika iziskuje definiranje jasnih dijagnostičkih kriterija. U liječenju se koriste nesteroidni protuupalni lijekovi (NSAID), glukokortikoidi, lijekovi koji modificiraju bolest (DMARD) i biološki lijekovi, a u novije vrijeme i inhibitori interleukina 17 i 23. Za neke od sindroma postoje jasni kriteriji za postavljanje dijagnoze dok je liječenje težih oblika bolesti još uvijek nedovoljno definirano i potrebna su dodatna ispitivanja koja će potvrditi učinkovitost pojedinih lijekova.
Abstract (english)
Autoinflammatory bone disorders are rare and their main feature is sterile bone inflammation accompanied by skin pathology. They encompass various syndromes: synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO syndrome), chronic recurrent multifocal osteomyelitis (CRMO), Majeed syndrome, cherubism and others. Syndromes often affect multiple organ systems and overlap with other diseases, so they often stay unrecognized. Early diagnosics require unambiguous definition of diagnostic criteria. Nonsteroidal anti-inflammatory drugs (NSAIDs), glucocorticoids, disease-modifying drugs (DMARDs) and biological drugs are used in treatment, and more recently, interleukin 17 and 23 inhibitors. Diagnostic criteria for some of the syndromes have been clearly established, while the treatment for more severe forms of disorders is still insufficiently defined. Further researches are needed to confirm the effectiveness of individual drug therapies.
