In this research, we analyzed the mutual relationship between the expression of MAGEA3 and the number of CD86 and CD163 positive intratumoral macrophages in pancreatic carcinomas. We also sought the connection between the clinical-pathological characteristics of the tumor and the patient's survival, which could lead to new diagnostic and prognostic indicators or potential targets for oncological therapy. The study aimed to analyze the immunohistochemical expression of MAGEA3 and the number of CD86 and CD163 positive intratumoral macrophages in pancreatic ductal adenocarcinomas. Specific objectives included comparing these results with the pathohistological characteristics of the tumor: tumor size, depth of invasion (pT stage), and the status of regional lymph nodes (pN stage). The research involved 76 subjects who were surgically treated for histopathologically confirmed adenocarcinoma of the pancreas at the Department of Surgery, UHC Sestre milosrdnice, from January 1st, 2003. until December 31st 2019. The patients were monitored postoperatively for at least 24 months (if there was no fatal outcome earlier), and their oncological status was recorded. Clinical data, postoperative course, and immunohistochemical analysis of tumor tissue were performed for each patient. The association of MAGEA3 antigen with other parameters included in the study, such as disease stage, patient survival, and the number of tumor-associated macrophages in the tumor tissue, was not proven. No significant correlation was found either with the T or N stage of the tumor. On the other hand, the number of CD86 intratumor macrophages and the CD86/CD163 ratio showed a statistically significant correlation with the increase in the T and N stages of the disease. MAGEA3 and TAMs may serve as prognostic markers or targets for oncology therapy, but further research is needed to exploit their potential. Surgical treatment remains the gold standard and the only potentially curative form of pancreatic adenocarcinoma treatment.
