Abstract Cilj: Glavni cilj ovog istraživanja je detaljno proučiti spolne razlike u histološkim značajkama suzne žlijezde čovjeka kroz kvalitativnu analizu histoloških i patohistoloških obilježja, stereološku analizu strukturnih elemenata žlijezda te analizu ekspresije AR i ER u tkivu žlijezde.
Materijali i metode: Na HE uzorcima 81 donora pod svjetlosni mikroskopom je provedena kvalitativna i patohistološka analiza. Na HE uzorcima 66 donora je provedena stereološka analiza volumenske gustoće sekretornog tkiva, vezivnog tkiva i masti prema načelima klasične sterologije. mRNA za analizu ekspresije AR, ERα i ERβ je izoliran iz 35 uzoraka tkiva ljudskih suznih žlijezda, a njihova ekspresija je kvantificirana s pomoću qPCR-a.
Rezultati: Kvalitativnom analizom ljudske suzne žlijezde svjetlosnim i elektronskim mikroskopom se ne mogu zamijetiti spolne razlike u strukturi. Iako su sve promatrane patohistološke promjene učestalije u ženskim uzorcima, samo atrofija acinusa je statistički značajno učestalija u ženskim, a dilatacija duktusa u muškim uzorcima. Stereološka analiza ukazuje na značajno manji udio acinusa u jedinici volumena lobulus ženskih ispitanika te značajno veći udio vezivnog i masnog tkiva u jedinici volumena žlijezde ženskih ispitanika. Dobno vezana degeneracija suznih žlijezda je izraženija kod ženskih ispitanika. Nisu dokazane spolne razlike u relativnoj ekspresiji mRNA receptora SSH odnosno utjecaj dobi na njihovu ekspresiju
Zaključak: Spolni dimorfizama suzne žlijezde čovjeka te značajnija korelacija degenerativnih promjena s porastom dobi u ženskih ispitanika bi mogli igrati važnu ulogu u većoj učestalosti DED kod starijih žena. Izostanak spolnih razlika u relativnoj ekspresiji mRNA receptora SSH odnosno izostanak utjecaja dobi na njihovu ekspresiju, sugerira veću važnost drugih čimbenika na razvoj disfunkcije suzne žlijezde. Dominacija ekspresije ERα u oba spola bi mogla ukazivati na ERα kao potencijalno važan cilj za buduća istraživanja selektivne hormonske terapije.
Abstract (english) Aim: The aim of this study was detailed analysis of sex-related differences in the histological features of the human lacrimal gland by means of qualitative analysis, stereological analysis, and AR and ER expression analysis.
Materials and methods: Qualitative histological analysis and pathohistological analysis were performed on HE samples from 81 donors. Stereological analysis of the volume density was performed on HE samples from 66 donors. mRNA for AR, ERα and ERβ expression analysis was isolated from 35 samples and quantified by qPCR.
Results: Qualitative analysis of the human lacrimal gland reveals no sex differences in its structure. Pathohistological analysis suggests higher frequency of acinar atrophy in female, and duct dilatation in male group. Stereological analysis confirms sexual dimorphism with a lower proportion of acini and higher proportion of connective and fatty tissue in female group. Age-related degeneration was more pronounced in female group. No sex-related or age-related differences in the relative expression of AR, ERα and ERβ mRNA have been found. ERα was dominant receptor in both sexes.
Conclusion: Sexual dimorphism and a more pronounced correlation of the age-related degenerative changes in female glands could play an important role in the higher frequency of DED in older women.
