Abstract
Kronična bolest jetre uzrokovana infekcijom virusa hepatitisa C (HCV) je jedan od najčešćih uzroka ciroze jetre i hepatocelularnog karcinoma diljem svijeta. Antivirusno liječenje s pegiliranim interferonom alfa (PEG-IFN α) i ribavirinom (RBV) dovodi do eliminacije virusa, tj. postizanja SVR-a (engl. Sustained virological response) u 40-80% bolesnika ; što ovisi o osobitostima bolesnika, ali i karakteristikama virusa. Uznapredovala životna dob jedan je od najvažnijih prediktora slabijeg odgovora na dvojnu terapiju. Cilj ovog retrospektivnog, kohortnog istraživanja bio je odrediti učinkovitost dvojne terapije s PEG-IFN α i ribavirinom (RBV) kod bolesnika starije životne dobi (≥60 godina). U studiju su uključeni svi bolesnici s kroničnim hepatitisom C u dobi ≥60 g. koji su liječeni dvojnom terapijom u Klinici za infektivne bolesti “Dr. Fran Mihaljević“ u Zagrebu u razdoblju od 2003. do 2013. g. U istraživanje je uključena i kontrolna skupina od 142 pacijenta s kroničnim hepatitisom C koji su praćeni u Klinici u razdoblju od 2005. do 2014. g. Ovo istraživanje je pokazalo utjecaj starije životne dobi na odgovor na liječenje, ishod i progresiju kroničnog hepatitisa C.
Abstract (english)
Chronic liver disease caused by Hepatitis C virus (HCV) infection is one of the most common causes of liver cirrhosis and hepatocellular carcinoma worldwide. Current dual treatment with pegylated interferon alpha (PEG-IFN α) and ribavirin (RBV) leads to viral elimination in 40-80% (SVR-sustained virological response) of the treated patients, depending on the host and HCV specificity. Advanced age as an important host specificity is in a correlation with lower response to dual antiviral therapy. The aim of this retrospective, cohort study was to evaluate the efficacy of dual antiviral therapy with PEG-IFN alpha + RBV in the elderly people (≥60 years). The study involved every patient (≥60 years; 2003-2013) treated with dual antiviral therapy and also a control group of the 142 patients with monitored HCV chronic infection at the Zagreb University Hospital for Infectious Diseases during the period (2005 -2014). This study summarizes the current known and less known facts in the elderly that influence the outcome. The elucidation of all those facts may facilitate better response to the therapy.
