Abstract Uvod i cilj: Adekvatne plazmatske razine tireoidnih hormona su iznimno bitne za rast, diferencijaciju i metabolizam svake stanice u tijelu. Dokazano je da brojni vanjski čimbenici utječu na hormonski status štitne žlijezde međutim vrlo je malo informacija o utjecaju prehrane na serumske razine fT3. Stoga je cilj ovog istraživanja bio odrediti utjecaj konzumiranja određenih skupina namirnica na razinu fT3 u serumu i na taj način identificirati skupine namirnica koje su u izravnoj vezi s razinom fT3 u zdravih pojedinaca.
Materijali i metode: U presječnu studiju su bili uključeni ispitanici stariji od 18 godina s područja grada Splita, otoka Visa i Korčule. Ukupan broj od 2640 ispitanika dobiven je nakon isključenja onih ispitanika koji su prijavili poremećaje u radu ili operacije štitnjače, koji uzimaju lijekove za regulaciju rada štitnjače te onih čije su vrijednosti fT3 bile izvan referentnog raspona. Koncentracija fT3 u plazmi određena je pomoću potpuno automatiziranog uređaja “Liaison” Biomedica Chemiluminescence Analyzer. Podaci su prikupljeni pomoću upitnika o učestalosti konzumacije pojedinih namirnica. Dobivene vrijednosti izražene su kao srednje vrijednosti, tj. aritmetička sredina ± standardna devijacija (SD). Pearsonov koeficijent korelacije je korišten za izračun brojčanih varijabli dok su t-test i ANOVA korišteni da bi se procijenila povezanost među grupama i vrijednostima fT3. Provedena je višestruka linearna regresijska analiza kako bi se procijenila povezanost prehrambenih faktora s razinom fT3 u plazmi.
Rezultati: U istraživanju smo uočili niže vrijednosti plazmatskog fT3 u žena u odnosu na muškarce. Linearna regresijska analiza pokazala je da su prehrambene skupine s visokim faktorom utjecaja za biljno ulje bile negativno povezane s plazmatskom razinom fT3 (β=-0.057, SE =0.011, p=.01). Prehrambena skupina s visoko – negativnim faktorom utjecaja za kruh od mekinja i pozitivnim faktorom utjecaja za bijeli kruh bile su pozitivno povezane s plazmatskom razinom fT3 (β=0.049, SE =0.011, p=.027) kao i skupina s visokim faktorom za čokoladu, kekse, kolače i bombone (β=0.044, SE =0.011, p=.049).
Zaključak: Učestala konzumacija biljnog ulja pokazala je negativnu povezanost s plazmatskom koncentracijom fT3 zbog boljeg staničnog iskorištenja fT3. Učestala konzumacija kruha od mekinja pokazala je negativnu povezanost s plazmatskom razinom fT3 zbog sniženog glikemijskog indeksa. Konzumacija prehrambenih namirnica s većim glikemijskim indeksom, kao što su bijeli kruh, čokolada, keksi, kolači i bomboni, pokazala je pozitivnu povezanost s plazmatskom konecntracijom fT3 zbog pretpostavke da hrana s visokim glikemijskim indeksom pojačava perifernu konverziju T4 u T3.
Abstract (english) Introduction and objectives: Adequate plasma levels of thyroid hormones are extremely important for the development, differentiation, and metabolism of every cell in the body. Numerous environmental factors have been shown to affect thyroid hormone status however there is very little information on the impact of diet on serum fT3 levels. Therefore, the objective of this study is to determine the impact of consuming certain groups of food items on serum fT3 levels and thus to identify food groups that are directly related to fT3 levels in healthy individuals.
Material and methods: This cross-sectional study was performed on adult participants over 18 years of age that originated from the city of Split and islands of Korčula and Vis. A total of 2640 participants were obtained after excluding those who reported thyroid disfunction or surgery, usage of medication, or fT3 levels outside the reference values. Plasma fT3 levels were determined using a fully automated instrument “Liaison” Biomedica Chemiluminescence Analyzer. Data were collected using a food frequency questionnaire. The data are presented as the mean ± standard deviation (SD). Pearson’s correlation coefficient was calculated for numerical variables, while t-test and ANOVA were used to assess the association between groups in fT3 levels. A multiple linear regression analysis was performed to assess the association of dietary factors with plasma fT3 levels.
Results: Linear regression analysis revealed that the dietary group with high loadings for plant oil was negatively associated with the plasma fT3 levels (β=-0.057, SE =0.011, p=.01). The dietary group with high-negative loading for bran bread and positive loading for white bread was positively associated with the plasma fT3 levels (β=0.049, SE =0.011, p=.027), as well as the dietary group with high loadings for chocolate, cookies, cakes, and bonbons (β=0.044, SE =0.011, p=.049). Other dietary groups showed no association with fT3 levels.
Conclusion: Frequent consumption of plant oil was negatively associated with plasma fT3 levels due to better cell utilization of fT3. Frequent consumption of bran bread showed a negative association with fT3 levels possibly due to a lower glycemic index. Consumption of food items with high glycemic index such as consumption of chocolate, cookies, cakes, and bonbons showed a positive association with fT3 levels suggesting that food with a high glycemic index stimulates T4 to T3 conversion.
