Abstract Cilj: Utvrditi učestalost pojavnosti bolesti PV i BP na području Splitsko-dalmatinske županije u desetogodišnjem periodu od 2012. do 2021. godine.
Materijali i metode: U retrospektivno istraživanje uključeno je 35 ispitanika kojima je patohistološki opisan PV i 182 ispitanika kojima je patohistološki opisan BP u periodu od 1. siječnja 2012. godine do 31. prosinca 2021. godine. Podatci su prikupljeni iz arhive Laboratorija za dermatološku patohistologiju Klinike za kožne i spolne bolesti KBC-a Split.
Rezultati: U razdoblju od 2012. do 2021. godine, zabilježena je incidencija PV od 0,9 / 100 000 stanovnika SDŽ godišnje. Incidencija BP bila je 5,3 / 100 000 stanovnika SDŽ godišnje. PV i BP podjednako su se dijagnosticirali u žena i u muškaraca. PV češće se dijagnosticirao u muškaraca (57%) s omjerom Ž:M=1:1,3, a BP u žena (55%) s omjerom M:Ž=1:1,2. Medijan dobi u kojoj su žene najčešće obolijevale od ovih bolesti iznosio je 75, a za muškarce 73 godine. Medijan dobi za PV iznosio je 70 (19-95) godina, a za BP 76 (26-100) godina. PV i BP podjednako su se često pojavljivali u dobnim skupinama do 59 godina, a u dobnim skupinama iznad 60 godina, BP se 7 puta češće dijagnosticirao nego PV. Najveći broj ispitanika dijagnosticiran je u 2016. (n=31), 2018. (n=30) i u 2021. godini (n=28 ispitanika), a najmanji je broj ispitanika zabilježen 2012. (n=11) i 2020. godine (n=14). Ovisnost bolesti PV i BP o godišnjim dobima nije dokazana iako bi se zbog geografskog položaja SDŽ mogao očekivati porast broja ispitanika u proljeće i ljeto (P=0,469).
Zaključak: Ovo istraživanje utvrdilo je da je učestalost PV-a u skladu s učestalošću u svijetu, dok je učestalost BP-a veća u SDŽ nego u ostalim dijelovima svijeta. Nije uočena statistički značajna razlika u pojavnosti PV i BP ovisno o spolu i godišnjem dobu. Medijan dobi za BP poklapa se s medijanom u svijetu, dok je medijan dobi za PV viši u SDŽ. 2020. godine zabilježen je očekivani pad broja ispitanika, dok 2021. nije došlo do naknadnog porasta broja ispitanika. Zbog visoke incidencije BP, potrebna su daljnja istraživanja kako bi se utvrdio razlog ove pojavnosti te bi istraživanje možda trebalo provesti na temelju podataka dobivenih DIF-om kako bi se dobio uvid u što realniju sliku učestalosti PV-a i BP-a u SDŽ.
Abstract (english) Objectives: The aim of this study was to determine the incidence of pemphigus vulgaris and bullous pemphigoid in the Split-Dalmatia County during the 10-year period from 2012 to 2021.
Material and Methods: The retrospective study included 35 patients with pathohistologically described PV and 182 patients with pathohistologically described BP in the period from January 1, 2012 to December 31, 2021 based on data collected from the medical records of the Histopathology Laboratory, Clinic of Dermatology and Venerology, University Hospital of Split.
Results: In the period from 2012 to 2021, the incidence of PV was 0.9/100,000 inhabitants of the Split-Dalmatia County per year. The incidence of BP was 5.3/100,000 inhabitants of the Split-Dalmatia County per year. PV and BP equally affected women and men. Men were more affected by PV (57%) with a female/male=1/1.3 ratio. On the other hand, women were more affected by BP (55%) with a female/male=1/1.2 ratio. The median age at which women were most often affected by these diseases was 75 and 73 for men. The median age for PV was 70 (range 19-95) years, and for BP 76 (range 26-100) years. PV and BP appeared equally often in age groups up to 59 years. However, in age groups over 60 years, BP was diagnosed 7 times more often than PV. The largest number of patients was diagnosed in 2016 (n=31), 2018 (n=30) and in 2021 (n=28), and the smallest number of patients was diagnosed in 2012 (n=11) and 2020 (n=14). The dependence of PV and BP diseases on the seasons has not been proven, although due to the geographical location of the Split-Dalmatia County an increase in the number of patients could be expected in spring and summer (P=0.469).
Conclusion: This study found that the incidence of PV is consistent with the incidence worldwide, while the incidence of BP is higher in the Split-Dalmatia County than in other parts of the world. No statistically significant difference was observed in the incidence of PV and BP depending on gender and season. The median age for BP matches the median age in the world, while the median age for PV is higher in the Split-Dalmatia County. In 2020, the expected decrease in the number was recorded, while in 2021 there was no subsequent increase in the number of patients. Due to the high incidence of BP, further research is needed to determine the reason for this occurrence. The research should perhaps be conducted based on medical records of DIF results in order to get more realistic picture of the incidence of PV and BP in the Split-Dalmatia County.
