Abstract Cilj: Cilj ovog rada bio je prikazati broj žena koje se odazivaju na preventivne preglede u Republici Hrvatskoj i Splitsko-dalmatinskoj županiji u razdoblju od 2000. do 2014. godine i pokazati razliku rezultata za vrijeme oportunističkog od nacionalnog probira. Ujedno je cilj bio prikazati pojavnost karcinoma vrata maternice od 2000. do 2011. godine u Republici Hrvatskoj i Splitsko-dalmatinskoj županiji te ih usporediti. Također, jedan od ciljeva, bio je vidjeti pojavnost raka vrata maternice u Republici Hrvatskoj i zemljama u okruženju od 1985. do 2012. godine.
Materijali i metode: Podaci su korišteni iz Zdravstveno-statističkog ljetopisa HZJZ od 2000. do 2014. godine te podaci o pojavnosti raka vrata maternice Republike Hrvatske dobiveni se iz Registra za rak Hrvatskog zavoda za javno zdravstvo i Zavoda za javno zdravstvo Splitsko-dalmatinske županije.
Rezultati: Rezultati su pokazali da u Republici Hrvatskoj i Splitsko-dalmatinskoj županiji od 2000. do 2014. godine broj žena koje su radile preventivne preglede za vrijeme oportunističkog probira lagano opadao, ali uvođenjem nacionalnog probira od 2012. godine broj preventivnih pregleda ponovno blago raste i broj patoloških nalaza u 2014. godini se udvostručio u odnosu na 2000. godinu. Ujedno je i 2014. godine, u ovom promatranom razdoblju, najveći broj patoloških nalaza. Pojavnost karcinoma vrata maternice u Republici Hrvatskoj je stagnirala do 2009. godine, zatim je naglo porasla te je 2011. godine bila najveća. U Splitsko-dalmatinskoj županiji uočava se niža pojavnost karcinoma vrata maternice. Incidencija raka vrata maternice u Republici Hrvatskoj niža je od većine zemalja u regiji, no još je uvijek viša od incidencije drugih zemalja, koje također imaju organizirane probire.
Zaključci: Podatci objavljeni u Zdravstveno statističkom ljetopisu HZJZ od 2000. do 2014. godine za Republiku Hrvatsku i Splitsko-dalmatinsku županiju pokazuju smanjenje odaziva žena na preventivne preglede od 2000. do 2011. godine. U zadnje tri godine, od uvođenja nacionalnog probira, broj patoloških nalaza povećao se za dva puta od 2000. godine. Pojavnost karcinoma vrata maternice u Republici Hrvatskoj, također je u porastu. Najveća pojavnost karcinoma bila je 2011. godine, što je 2,5 puta veći broj nego 2000. godine. U Splitsko-dalmatinskoj županiji je niža pojavnost karcinoma vrata maternice nego u Republici Hrvatskoj. Najveća stopa smrtnosti u zemljama u okruženju Republike Hrvatske je u Mađarskoj, a najniža stopa je u Austriji. Prevencija od infekcije HPV-om sastoji se od edukacije o spolnom ponašanju, redovitu uporabu zaštite, cijepljenje protiv HPV-infekcije, redovite liječničke kontrole te uvođenje organiziranog probira.
Abstract (english) Objective: The objective of this study was to show the number of women who respond to preventive examinations in the Republic of Croatia and Split-Dalmatia County in the period from 2000 - 2014 and show the difference results during opportunistic than the national screening. At the same time the aim was to show the incidence of cervical cancer in period of 2000 - 2011 in the Republic of Croatia and Split-Dalmatia County and compare them. Also, one of the goals was to see the incidence of cervical cancer in the Republic of Croatia and the neighboring countries in period of 1985 - 2012.
Materials and Methods: Data were used from the Health Statistical Yearbook HZJZ from 2000 to 2014 and data on the incidence of cervical cancer in Croatia obtained from the Cancer Registry of the Croatia Institute for Public Health and the Public Health Institute of Split-Dalmatia County.
Results: The results showed that in the Republic of Croatia and Split-Dalmatia County from 2000 to 2014 the number of women working preventive examinations during opportunistic screening slightly declined, but the introduction of a national screening of 2012, the number of preventive examinations again slightly increases and number of pathological findings in 2014 doubled in comparison with 2000. At the same time, in 2014, in the observed period, the largest number of pathological findings. The incidence of cervical cancer in Croatia stagnated until 2009, then increased rapidly and in 2011 was the largest. In Split-Dalmatia County can be seen lower incidence of cervical cancer. The incidence of cervical cancer in Croatia is lower than most countries in the region, but it is still higher than the incidence of other countries, which also have organized screening.
Conclusions: The data published in Health Statistics Annals HZJZ from 2000 - 2014 for the Republic of Croatia and Split-Dalmatia County show a decrease in the response of women in preventive examinations from 2000 - 2011. In the last three years, since the introduction of national screening, pathological findings increased twice since 2000. The incidence of cervical cancer in Croatia, is also increasing. The highest incidence of cancer was in 2011, which is 2.5 times more than in 2000. In Split-Dalmatia County is the lower incidence of cervical cancer than in Croatia. The highest mortality rates in the neighboring countries of the Republic of Croatian in Hungary, while the lowest rate was in Austria. Prevention of HPV infection consists of education on sexual behavior, regular use of protection, vaccination against HPV infection, the usual medical supervision and implementation of an organized screening.
