Abstract Ciljevi istraživanja: Ispitati koji su najčešći simptomi Menierove bolesti u odnosu na stadije bolesti te utvrditi postoje li razlike u karakteristikama simptoma u odnosu na dob i spol. Utvrditi prosječan gubitak sluha svih ispitanika te prosječan gubitak sluha i najzastupljeniji tip krivulje tonske audiometrije u odnosu na stadije bolesti. Ispitati vestibularnu funkciju. Materijali i metode: Materijali su specijalistički ORL nalazi te nalazi tonske audiometrije i vestibulometrije iz registra Zavoda za audiologiju. Uključeni su samo bolesnici s definitivnom MB. Ispitanici su podijeljeni u dvije skupine ovisno o spolu (92 žene i 40 muškaraca) i dobi (42-62 i 63-82 godine) te u četiri skupine ovisno o stadiju bolesti utvrđenom prema nalazu tonske audiometrije. Rezultati: Utvrđena je značajna povezanost svih simptoma, osim punoće u uhu (P=0,127), sa stadijem bolesti. U prvom i drugom stadiju najčešći su simptomi vrtoglavica, fluktuacija sluha i šum u uhu, a u trećem stadiju vrtoglavica, šum u uhu i punoća u uhu. U četvrtom stadiju najčešći simptom je šum u uhu. Prosječan gubitak sluha svih ispitanika iznosi 50,38 ± 23,64 dB. U prvom i drugom stadiju najčešće je prisutan uzlazni tip krivulje, dok je u trećem i četvrtom stadiju najčešće prisutan ravni tip. Mlađa skupina ima značajno veću učestalost napadaja vrtoglavice (P<0,001) i fluktuacija sluha (P<0,001), dok starija ima češće prisutnu vestibularnu hipofunkciju (P<0,001) te veći prosječan gubitak sluha (72,79 dB) u odnosu na mlađu dobnu skupinu (31,13 dB). Bilježe se razlike u tipu krivulje u odnosu na spol i dob (P<0,001). Ne bilježe se značajne razlike u učestalosti simptoma u odnosu na spol (P>0,005). Zaključci: S progresijom bolesti i starenjem uočava se smanjenje učestalosti fluktuacija sluha i vrtoglavica te povećanje prosječnog gubitka sluha i učestalosti vestibularne hipofunkcije. Uzlazni tip krivulje češći je u ranijim stadijima te u mlađih ispitanika, dok je ravni tip češći u kasnijim stadijima i u starijih ispitanika.
Abstract (english) Research objectives: The research objectives were to examine which are the most common symptoms of Meniere's disease in relation to the stages of the disease and to determine whether there are differences in the characteristics of the symptoms in relation to age and gender. We determined the average hearing loss of all subjects and the average hearing loss and the most common type of tone audiometry curve in relation to the stages of the disease. We examined the vestibular function in all patients. Materials and methods: The materials are specialist ENT findings as well as findings of pure tone audiometry and vestibulometry from the register of the Institute of Audiology. Only patients with a definite MD were included in the study. The subjects were divided into two groups depending on gender (92 women and 40 men) and age (42-62 and 63-82 years), and into four groups depending on the stage of the disease determined by pure tone audiometry findings. Results: Statistical analysis revealed a significant association of all symptoms, except aural fullness (P=0.127), with the stage of the disease. In the first and second stages the most common symptoms are vertigo, hearing fluctuations and tinnitus, and in the third stage vertigo, tinnitus and aural fullness. In the fourth stage, the most common symptom is tinnitus. The average hearing loss of all subjects was 50.38 ± 23.64 dB. In the first and second stages, the ascending type of curve is the most common, while in the third and fourth stages, it is the flat type. The younger group has a significantly higher frequency of vertigo attacks (P<0.001) and hearing fluctuations (P<0.001), while the older group has more frequent vestibular hypofunction (P<0.001) and a higher average hearing loss (72.79 dB) compared to the younger group (31.13 dB). There are differences in curve types in relation to gender and age (P<0.001). There are no significant differences in the frequency of symptoms in relation to gender (P>0.005). Conclusion: With the progression of the disease and aging we see a decrease in the frequency of hearing fluctuations and vertigo and an increase in the average hearing loss and the frequency of vestibular hypofunction. The ascending type of curve is more common in earlier stages and in younger subjects, while the flat type is more common in later stages and in older subjects.
