Abstract | Cilj istraživanja: Svrha ovog istraživanja bila je usporediti plazmatske koncentracije adropina u bolesnika s opstrukcijskom apnejom tijekom spavanja (OSA) i kontrolnih ispitanika.
Materijali i metode: Ovo presječno kliničko istraživanje provedeno je u Centru za medicinu spavanja Kliničkog bolničkog centra Split i Medicinskog fakulteta u Splitu tijekom razdoblja od 1. listopada 2016. do 1. lipnja 2017. godine, a obuhvatilo je ukupno 28 odraslih novodijagnosticiranih bolesnika s OSA-om i 21 kontrolnih ispitanika usklađenih prema ITM-u, dobi i spolu. U dijagnozi OSA-e korištena je kontinuirana cjelonoćna laboratorijska polisomnografija (Alice 5LE, Philips Respironics, Eindhoven, Nizozemska) prema dijagnostičkim kriterijima Američke akademije za medicinu spavanja (AASM) iz 2014. godine te Europskog društva za istraživanje spavanja (ESRS). Svi ispitanici podvrgnuti su detaljnoj anamnezi, fizikalnom pregledu i mjerenjima antropometrijskih obilježja. Iz venskih uzoraka krvi ispitanika natašte određene su plazmatske koncentracije adropina i drugi biokemijskih parametara. Plazmatske koncentracije adropina određene su koristeći dvostruki enzim-imunoadsorpcijski test (engl. enzyme-linked immunosorbent assay - ELISA) humanog adropina (Phoenix Pharmaceuticals, Burlingame, CA, SAD), prema uputama proizvođača.
Rezultati: Nisu pronađene statistički značajne razlike u promatranim biokemijskim parametrima između kontrolne i OSA skupine, osim u vrijednostima HDL-kolesterola koji je bio značajno niži u OSA bolesnika (1,1 ± 0,2 vs. 1,4 ± 0,2 mmol/L; P<0,001). Srednja vrijednost apneja hipopneja indeksa (AHI) u OSA bolesnika bila je 40,5 ± 18,1 (P<0,001). Plazmatske koncentracije adropina statistički su značajno niže u OSA bolesnika u odnosu na kontrolnu skupinu (4,0 ± 1,1 vs. 7,5 ± 2,0 ng/mL; P<0,001). Pronađena je statistički značajna negativna korelacija vrijednosti AHI (r=-0,498; P=0,007) i desaturacijskog indeksa (ODI) (r=-0,417; P=0,027) te pozitivna korelacija HDL-kolesterola (r=0,375; P=0,049) s plazmatskim koncentracijama adropina u OSA bolesnika.
Zaključci: Naše istraživanje pokazalo je da su plazmatske koncentracije adropina značajno niže u OSA bolesnika u usporedbi s kontrolnom skupinom. Nadalje, plazmatske koncentracije adropina negativno koreliraju s AHI vrijednosti i desaturacijskim indeksom. |
Abstract (english) | Objectives: The aim of this study was to compare plasma adropin concentrations between patients suffering from obstructive sleep apnea (OSA) and control group.
Materials and methods: This cross-sectional study was conducted at the Split Sleep Medicine Center from 1st of October 2016 to 1st of June 2017, and included totally 28 adult newly diagnosed patients with OSA and 21 control subjects matched for body mass index, age and sex. Diagnosis of obstructive sleep apnea was established by all night in-laboratory polysomnography (Alice 5LE, Philips Respironics, Eindhoven, Netherlands) according to the diagnostic criteria of American academy of sleep medicine (AASM, 2014) and European sleep research society (ESRS). All subjects underwent detailed anamnesis, physical examination and anthropometric measurements. Venous blood samples were taken from fasting patients in order to determine plasma adropin concentrations and other biochemical parameters. Plasma adropin concentrations were determined by an enzyme-linked immunosorbent assay (ELISA) method using an appropriate kit for human adropin (Phoenix Pharmaceuticals, Burlingame, CA, SAD), according to the manufacturer's instructions.
Results: Statistically significant differences weren't found in biochemical parameters between OSA and control group, except in HDL-cholesterol which was significantly lower in OSA subjects (1.1 ± 0.2 vs. 1.4 ± 0.2 mmol/L; P<0.001). Mean apnea-hypopnea index (AHI) in OSA subjects was 40.5 ± 18.1 (P<0.001). Plasma adropin concentrations are significantly lower in OSA subjects compared to control group (4.0 ± 1.1 vs. 7.5 ± 2.0 ng/mL; P<0.001). Statistically significant negative correlation was found between AHI (r=-0.498; P=0.007) and oxygen desaturation index (ODI) (r=-0.417; P=0.027) values and positive correlation of HDL-cholesterol (r=0.375; P=0.049) with plasma adropin concentrations in OSA subjects.
Conclusions: Our study showed that plasma adropin concentrations are significantly lower in OSA subjects compared to control group. Furthermore, there is negative correlation between plasma adropin concentration and values of AHI and ODI. |
