| Abstract | Cilj istraživanja: Cilj ovog istraživanja bio je ispitati učestalost i obilježja realimentacijskog sindroma u djece i adolescenata s anoreksijom nervozom, uz opis medicinskih intervencija i dijetetskih mjera, kao i rezultata primjenjivanog liječenja.
Materijali i metode: Za izradu ovog istraživanja retrospektivno su analizirani podatci 140 pacijenata hospitaliziranih pod dijagnozom AN ili EDNOS u razdoblju od 1.siječnja 2014. do 1.lipnja 2019. godine. Podatci su prikupljeni iz digitalne arhive službene bolničke medicinske dokumentacije Centra za poremećaje u jedenju djece i adolescenata KBC-a Sestre milosrdnice u Zagrebu te pisanog protokola i pismohrane povijesti bolesti Odjela za gastroenterologiju Klinike za pedijatriju KBC-a Split.
Rezultati: U studiju je bilo uključeno ukupno 140 ispitanika prosječne životne dobi od 14,9 ± 1,8 godina. Zabilježena je pojava realimentacijskog sindroma u 31 (22%) ispitanika, dok je 37% ispitanika imalo laboratorijske vrijednosti fosfata 1,07 mmol/L uz 95%CI: 27-49%. Medijan postotka idealne tjelesne mase (%IBW) je za 5% manji (r=0,22; P=0,009), dok je prosječna vrijednost indeksa tjelesne mase (ITM) za 0,9 kg/m2 manja (P=0,019) u skupini ispitanika s dijagnosticiranim RS. Nismo dokazali statistički značaj pojave RS u ispitanika hranjenih putem nazogastrične sonde (P=0,102). Medijan serumskih koncentracija fosfata pri početku realimentacije (P1) je za 0,26 mmol/L manji u skupini ispitanika s RS u odnosu na skupinu bez RS (r=0,42; P<0,001) dok se pri kraju hospitalizacije koncentracije fosfata (P2) nisu statistički značajno razlikovale (P=0,281). Došlo je do povećanja medijana vrijednosti ALT (r=0,36; P<0,001) i AST (r=0,45; P<0,001) pri prijemu te tijekom realimentacije (ALT (r=0,65; P<0,001); AST (r=0,66; P<0,001)), u ispitanika s RS.
Zaključak: Realimentacijski sindrom nije rijetka pojava u bolesnika s poremećajima u jedenju, a karakteriziran je brojnim kliničkim znakovima i simptomima uz popratni elektrolitni disbalans od kojeg se posebice izdvaja hipofosfatemija. Ostaje potreba za daljnjim dobro dizajniranim istraživanjima na većem uzorku ispitanika kako bi se odredila stvarna incidencija RS te razlikovanje od realimentacijske hipofosfatemije jer upravo zbog manjka jasne i uniformne definiranosti RS nastaju poteškoće pri dijagnostici, a posljedično i terapiji. Ključ uspješnog liječenja čini isključivo integrirani multidisciplinarni individualizirani pristup oboljelima. |
| Abstract (english) | Objectives: The aim of this study was to demonstrate the incidence and the characteristics of refeeding syndrome in children and adolescents with anorexia nervosa, with a description of the medical interventions and dietary measures, as well as the results of the applied treatment.
Materials and methods: This retrospective research study analyzed 140 patients hospitalized with the diagnosis of AN or EDNOS during the period from 1st January 2014 to 1st June 2019. The data were collected from the digital archive of the official hospital medical documentation of the Department of Gastroenterology of the „Sestre Milosrdnice“ University Hospital in Zagreb and the written protocol and the history from the archives of the Department of Gastroenterology of the University Hospital in Split.
Results: The study included 140 examinees average age of 11,9 ± 10,4 months. The occurrence of refeeding syndrome was observed in 31 (22%) examinee and the 37% examinee had laboratory phosphate values 1,07 mmol/L with 95%CI: 27-49%. The median percentage of ideal body weight (% IBW) was 5% less (r = 0.22; P = 0.009), while average body mass index (BMI) was 0.9 kg/m2 less (P = 0.019) in group of patients with diagnosed RS. We did not find the statistical significance of RS in those fed with nasogastric tube (P=0,102). The median concentration of serum phosphates at the beginning of the refeeding (P1) was 0,26 mmol/L lower in the patients with RS compared to the patients without RS (r=0,42; P<0,001) while at the end of the hospitalization phosphate concentration (P2) did not differ significantly (P=0,281). There was an increase in median ALT (r=0,36; P<0,001) and AST (r=0,45; P<0,001) at reception and during the refeeding process (ALT (r=0,65; P<0,001); AST (r=0,66; P<0,001)), in patients with RS.
Conclusion: The refeeding syndrome is not a rare phenomenon in patients with eating disorders and has been characterized by numerous clinical signs and symptoms associated with the electrolyte imbalance, especially hypophosphatemia. Remains a need for further, well-designed research on a larger sample of examinees do determine the real incidence of RS and distinction of the RS from the refeeding hypophosphatemia because due to the lack of clear and uniform definition of RS confusion arises in diagnosis and consequently in therapy. The key to successful treatment is only the integrated multidisciplinary individualized approach to the patients. |
