Abstract
Cilj. Prikazati Barthel i FIM indeks u evaluaciji rehabilitacije bolesnika s preboljelim moždanim udarom, usporediti njihove prednosti i nedostatke te objasniti važnost pravilno ispunjene fizioterapijske procjene. Metode i ispitanici. U kratkom uvodnom dijelu obradila sam činjenice poznate o cerebrovaskularnom inzultu, njegovu klasifikaciju, simptome koji ga karakteriziraju, faktore rizika bitne za njegov nastanak te rehabilitaciju nakon preboljelog cerebrovaskularnog inzulta. U drugom dijelu izneseni su podatci o Barthelu i FIM-u, indeksima koji ocjenjuju razinu onesposobljenosti ili preostalu funkcijsku sposobnost bolesnika s neurološkim oštećenjem. Pomoću ova dva validna indeksa, na primjeru ispitanice, iznesena je važnost pravilne fizioterapijske procjene i uključivanja bolesnika u proces rehabilitacije. Rezultati. Barthelov indeks mjeri samo motorno funkcioniranje, a FIM indeks mjeri motorno i kognitivno funkcioniranje. Svaki od njih je pažljivo dizajniran i specifičan za pojedinu bolest. FIM indeks je opsežnija i osjetljivija mjera funkcionalne onesposobljenosti nego Barthelov indeks. Međutim, zbog prirode posla i vremena potrebnog za rehabilitaciju na akutnim odjelima i ustanovama za održavajuću rehabilitaciju, u tom kratkom periodu korištenje FIM-a nije adekvatno. Iz navedenih razloga se koriste upitnici koji daju uvid u osnovne teškoće i onesposobljenost bolesnika, kao što je Barthel indeks. Zaključci. FIM daje puno jasniju sliku o funkcionalnom stanju bolesnika, pri čemu možemo zaključiti o njegovom razumijevanju i izražavanju te o socijalnim kontaktima, rješavanju problema i pamćenju, što je nedostatak Barthel indeksa. Međutim, zbog kraće evaluacije i planiranja rehabilitacijskih postupaka, Barthelov indeks se može koristiti zbog specifičnosti prilika.
Abstract (english)
Objective. To present Barthel index and FIM in evaluation of the rehabilitation of patients with history of stroke, compare their advantages and disadvantages and explain the importance of properly completed physiotherapy evaluation. Methods and patients. In the introduction I explained someknown facts about stroke, classification of stroke, characterizing symptoms, risk factors important to stroke appearance and rehabilitation after stroke. In the second part, we have informations about Barthel and FIM, two indices that evaluate the level of disability or residual functional capacity of patients with neurological damage. With these two valid indices, shown on the example of patient, we have presented the importance of properly physiotherapy evaluation and inclusion of patients in the rehabilitation process. Results. Barthel index measures only motor function, and FIM measures motor and cognitive functions. Each of them is carefully designed and specific to a particular disease. FIM is more comprehensive and more sensitive measure of functional disability then Barthel index. However, because of the type of the work and the time needed for rehabilitation, at acute care departments and in sustaining rehabilitation institutions, use of FIM is not adequate. Because of that, we use questionnaires which gave insight into the basic problems and disabilities of patients, like Barthel index. Conclusion. FIM gives much better show of the functional state of the patient, whereby we can make conclusions about its understanding and expression, social contacts, problems solving and memory, which is deficiency of Barthel index. However, because of shorter evaluation and planning of rehabilitation, Barthel index can be used in specific occasions.
