Abstract Cerebralna paraliza je jedan od najučestalijih kongenitalnih problema te se definira kao grupa neprogresivnih ali često promjenjivih motoričkih oštećenja uzrokovana lezijom središnjeg živčanog sustava u ranim stadijima razvoja. Usprkos napretku perinatalne zdravstvene zaštite, incidencija ostaje konstanta gdje na 1000 živorođene novorođenčadi 2-3,5 je rođeno s CP.
Motorička oštećenja nerijetko su praćena sekundarnim oštećenjima. Smetnje vida, smetnje ponašanja te kognitivni poremećaji uz epilepsiju jedan su od najčešćih pridruženih odstupanja.
Najviši potencijal za učenje i prilagodbu na cerebralnu paralizu prisutan je upravo tijekom prvih 18 mjeseci života zbog čega je rana dijagnoza od iznimne važnosti. Što ranija dijagnoza omogućuje veću šansu za bolji neuromotorni ishod. Koriste se brojni klinički i laboratorijski dijagnostički postupci kako bi se na vrijeme otkrila neurorazvojna odstupanja. Dodatno otežava dijagnozu to što simptomi ne nastaju odmah, već se razvijaju s vremenom. Potrebno je na vrijeme procijeniti spontane pokrete, neonatalne primitivne reflekse te položajne reakcije.
Bez obzira na oblik CP, promijenjen mišićni tonus uvijek je prisutan. Bilo da se radi o povišenom ili smanjenom mišićnom tonusu, dolazi do smanjenja opsega pokreta, što je u kasnijem razvoju glavni uzrok nastalih kontraktura ili deformacija. Glavni ciljevi terapije svode se na postizanje opsega pokreta u djeteta koje će mu omogućiti najbolju moguću lokomocijsku funkciju, preveniranje razvoja deformacija te korigiranje abnormalnih obrazaca položaja i pokreta koje su uzrokovane neurološkom disfunkcijom.
Potrebno je što više poticati razvoj djetetovih funkcionalnih sposobnosti koje će mu omogućiti maksimalnu neovisnost u aktivnostima svakodnevnog života.
Abstract (english) Cerebral Palsy is one of the most common congenital problems and is defined as a group of non-progressive but often variable motor damage caused by the central nervous system lesion in early stages of development. Despite the progress of perinatal health care, the incidence remains a constant, where 1000 newborn babies 2-3,5 were born with CP.
Motor damage is often accompanied by secondary damage. Visual disturbances, behavioral disorders, and cognitive disorders associated with epilepsy are one of the most common associated deviations.
The highest potential for learning and adaptation to cerebral palsy is present during the first 18 months of life, which is why early diagnosis is of the utmost importance. The earlier diagnosis provides a better chance for a better neuromotor outcome. Numerous clinical and laboratory diagnostic procedures are used to detect neurodevelopmental deviations on time. What makes it more difficult to diagnose is that symptoms do not occur right away, but are developing over time. It is necessary to evaluate spontaneous movements, neonatal primitive reflexes and positional reactions in time.
Regardless of the shape of CP, modified muscle tone is always present. Whether it is an elevated or decreased muscle tone, there is a reduction in the extent of motion, which is in later development the main cause of contractures or deformations. The main goals of the therapy are to achieve the extent of motion of the child that would provide the best possible locomotional function, to prevent deformation development, and to correct abnormal position and motion patterns caused by neurological dysfunction.
It is necessary to encourage more the development of the child's functional abilities that will enable him the maximum independence in the activities of everyday life.
