Abstract Idealno bi bilo da se svaka trudnica kod koje se predvidi rođenje djeteta sa visokim rizikom, prije početka porođaja transportira „in utero“ u ustanovu odgovarajuće razine. Novorođenče koje je životno ugroženo, a rođeno je u centru s nižim stupnjem perinatalne zaštite mora se transportirati u viši centar perinatalne zaštite. Zbrinjavanje životno ugrožene novorođenčadi započinje u rađaonici ili operacijskoj sali. Pravodobnim liječenjem sprječavaju se sekundarna oštećenja te se djetetu omogućava bolja kvaliteta života. Prijevremeno rođena novorođenčad, novorođenčad s prirođenim anomalijama organa i sustava, novorođenčad sa sepsom, konvulzijama i metaboličkim bolestima spadaju u životno ugroženu novorođenčad i zahtijevaju medicinsko-intenzivnu skrb kao i poduzimanja postupaka usmjerenih ka spašavanju njihovih života.
U KBC-u Split u 2015. godini registrirano je ukupno 4226 poroda, a od toga 4449 ukupno rođene djece, te 4432 živorođena djeteta. Od tog broja šestoro djece rođeno je sa dijagnozama teških srčanih anomalija koja su medicinski transportirana u više centre radi daljnjeg liječenja. Jedno od njih je muško novorođenče K. J. koje je po rođenju primljeno na Klinički odjel za neonatologiju, u jedinicu za intenzivno liječenje novorođenčadi gdje se odmah pristupilo hitnim medicinskim intervencijama u cilju stabilizacije djetetova stanja. Nakon potvrđene medicinske dijagnoze teške srčane anomalije koja zahtjeva operativno liječenje, pristupa se brzom organiziranju i pripremama za transport novorođenčeta u viši centar, u kojem medicinska sestra ima važnu i odgovornu ulogu. Tijekom samog medicinskog transporta od medicinske sestre se očekuje maksimalna savjesnost i odgovornost. Treba biti iskusna i educirana kako bi na vrijeme znala prepoznati znakove određenih komplikacija, pravovremeno reagirala i primijenila postupke u okviru svojih kompetencija.
Abstract (english) It would be ideal for every pregnant woman, whose child is supposed to be born with high risk, to be transported „in utero“, before childbirth, in appropriate institution. Life threatening newborn that is born in the centre with lower grade of perinatal protection must be transported in the centre with higher grade of perinatal protection. Care about life threatening newborn begins in delivery room or operational room. With timely treatment secondary damage is being hindered and child is being provided with better quality of life. Preemies, newborns with congenital anomalies of organs and systems, newborns with sepsis, convulsions and metabolic disease are considered as life threatening. They require medical-intensive care and actions that save their lives.
In Clinical Hospital Centre in Split, during 2015., 4226 births are registered. Altogether 4449 children are born, thereof 4432 alive. 6 of those children are born with diagnoses of serious heart defects and are transported to centres with higher grade of perinatal protection because of further treatment. 1 of those children is male newborn K.J. After birth he was settled in Neonatology Clinical Care Unit, more precisely Intensive Care Unit. To stabilize childs life, neonatology team immediately started with urgent medical care. After diagnose of serious heart defect, that requires urgent medical operation, was affirmed, newborns transport was organized and prepared rapidly. During transport to center with higher grade of perinatal protection, nurse has an important and responsible role. Nurse is supposed to be very conscientious and responsible. She has to be skilled and educated in order to recognize sympthoms of complications and to react in time.
