Abstract Uvod: Monoklonska gamapatija je imunoproliferativna bolest koju karakterizira prisutnost monoklonskog proteina u serumu ili urinu, a nastaje kao produkt klona limfocita B koji je genetički promijenjen. Najčešći monoklonski protein je iz klase G imunoglobulina. Maligna monoklonska gamapatija očituje se kao multipli mijelom, to jest, diseminirani zloćudni tumor građen od monoklonskih plazma stanica. Bence-Jonesovi proteini su monoklonski slobodni laki lanci imunoglobulina, kapa i lambda, koje stvara maligni klon B-limfocita te su kao takvi važan tumorski biljeg u dijagnostici multiplog mijeloma. Elektroforeza serumskih proteina je prvi korak u laboratorijskoj dijagnostici monoklonskih gamapatija, a danas je najzastupljenija u praksi kapilarna zonska elektroforeza. Serumski proteini razdvajaju se u pet glavnih frakcija: albumini, alfa-1 globulini, alfa-2 globulini, beta globulini i gama globulini. U slučaju nalaza sumnjive monoklonske vrpce u nalazu elektroforeze serumskih proteina, potrebno je učiniti imunofiksaciju seruma kako bi potvrdili prisutnost te odredili razred i tip monoklonskog proteina.
Cilj rada: Svrha rada bila je metodom imunofiksacije ispitati prisutnost monoklonskih proteina u serumima pacijenata sa suspektnim M-vrškom u nalazu elektroforeze serumskih proteina.
Materijali i metode: Izabrana su četiri uzorka pacijenata zaprimljena u Zavodu za medicinsko laboratorijsku dijagnostiku KBC-a Split. Kapilarnom elektroforezom serumskih proteina, u sva četiri uzorka, uočena je oštra vrpca u gama regiji što je upućivalo na prisutnost M-proteina. Odabrani uzorci seruma podvrgnuti su imunofiksaciji.
Rezultati: U sva četiri ispitivana uzorka dokazana je prisutnost monoklonskog proteina. Metodom imunofiksacije dokazani su monoklonski proteini IgG kapa tipa u sva četiri ispitivana seruma.
Zaključak: Metoda imunofiksacije potvrdila je prisustvo M-proteina IgG kapa tipa u sva četiri seruma sa prethodno postavljenom sumnjom na temelju uočenog karakterističnog vrška na elferogramu serumskih proteina.

Abstract (english) Introduction: Monoclonal gammopathy is an immunoproliferative disease characterized by the presence of monoclonal protein in serum or urine, produced by the clone of lymphocyte B that is genetically transformed. The most common monoclonal protein is class of immunoglobulin G. Malignant monoclonal gammopathy is manifested mostly as a multiple myeloma, disseminated malignant tumor composed of monoclonal plasma cells. Bence-Jones proteins are monoclonal free light chains of immunoglobulins, kappa or lambda, which are produced by the malignant clone of B lymphocytes as a part of monoclonal immunoglobulin. Bence Jones proteins are an important tumor marker in the diagnosis of multiple myeloma. Serum protein electrophoresis is a method for detection of monoclonal protein. Today the most common method for serum electrophoresis is capillary zone electrophoresis. Serum proteins are separated into five major fractions: albumin, alpha-1 globulin, alpha-2 globulin, beta globulin and gamma globulin. If the monoclonal spike is found by electrophoresis, immunofixation should be performed to confirm the presence and determine the class and type of monoclonal protein.
Objective: The purpose of the study was to confirm the presence of monoclonal proteins in the serum of the patients with M spike by the immunofixation electrophoresis as well as to define the class and type of monoclonal protein.
Materials and methods: Samples of four patients accepted at the Department of Medical Laboratory Diagnosis, UHC Split were selected for immunofixation testing considering the suspected M spike found by serum proteine electrophoresis.
Results: In all four samples the presence of monoclonal proteine was confirmed. The monoclonal IgG kappa type was determined in all four samples.
Conclusion: Immunofixation testing confirmed the presence of monoclonal proteine IgG kappa type in every selected sample with M spike found by serum electrophoresis.
