Abstract Cilj. Odrediti učestalost bolesnika s neurološkim bolestima kod kojih se provodi fizikalna terapija u kući u Splitsko-dalmatinskoj županiji te odrediti zastupljenost pojedinih neuroloških bolesti, broj i dužinu trajanja procedura po dijagnozama. Odrediti učestalost akutne i održavajuće rehabilitacije. Evaluirati uspješnost rehabilitacije koristeći Barthelov indeks za procjenu funkcionalnog statusa.
Metode. Podaci o bolesnicima kod kojih je provedena fizikalna terapija u kući, u vremenskom periodu od siječnja do prosinca 2019.godine, prikupljeni su iz registra Ustanove za zdravstvenu njegu i rehabilitaciju Vita. Kod prikupljanja podataka korišten je opći upitnik koji je sadržavao podatke o dijagnozi, dobi, spolu, broju procedura i obliku terapije, a za procjenu funkcionalnog statusa korišten je Barthelov indeks.
Rezultati. Fizikalna terapija u kući provedena je kod 1093 bolesnika. Neurološku bolest su imala 544 (49,77%) bolesnika, od toga 52.57% muškaraca i 47,43% žena. Najveći broj bolesnika je starosne dobi od 71-80 godina. Udio neuroloških bolesti značajno je veći u odnosu na druge bolesti. Među neuroloških bolestima najzastupljenija je cerebralna paraliza i ostali paralitički sindromi. Kod oboljelih od demijelinizirajućih bolesti središnjeg živčanog sustava proveden je značajno veći broj procedura fizikalne terapije u kući, u odnosu na oboljele od drugih neuroloških bolesti. Održavajuću rehabilitaciju imalo je 65,44% bolesnika, što je značajno više od broja bolesnika koji su imali akutnu rehabilitaciju (χ2=51,88; p<0,001). Zabilježen je značajan porast vrijednosti Barthelova indeksa nakon provedene rehabilitacije u kući bolesnika (Z=19,08; p<0,001).
Zaključci. Najbrojniji bolesnici kod kojih se provodila fizikalna terapija u kući bili su bolesnici s neurološkim bolestima. Među neurološkim bolestima najviše je bolesnika s cerebralnom paralizom i ostalim paralitičkim sindromima (G80-G83, MKB10). Održavajuća rehabilitacija bila je zastupljenija od akutne rehabilitacije. Najveći broj procedura proveden je kod bolesnika s demijelinizirajućim bolestima živčanog sustava. Značajnim povećanjem vrijednost Barthelova indeksa dokazana je uspješnost rehabilitacije u kući .
Abstract (english) Aim. Determine the frequency of patients with neurological diseases in whom physical therapy is performed at home in the Split-Dalmatia County and determine the prevalence of individual neurological diseases, the number and duration of procedures by diagnosis. Determine the frequency of acute and maintenance rehabilitation. Evaluate the success of rehabilitation using the Barthel index to assess functional status.
Methods. Data on patients who underwent physical therapy at home, in the period from January to December 2019, were collected from the register of the Vita Health Care and Rehabilitation Institution. A general questionnaire containing data on diagnosis, age, sex, number of procedures and form of therapy was used in data collection, and the Barthel index was used to assess functional status.
The results. In-home physical therapy was performed in 1093 patients. 544 (49.77%) patients had neurological disease, of which 52.57% were men and 47.43% were women. The largest number of patients is aged 71-80 years. The proportion of neurological diseases is significantly higher compared to other diseases. Among neurological diseases, cerebral palsy and other paralytic syndromes are the most common. In patients with demyelinating diseases of the central nervous system, a significantly higher number of physical therapy procedures were performed at home, compared to patients with other neurological diseases. 65.44% of patients had maintenance rehabilitation, which is significantly more than the number of patients who had acute rehabilitation (χ2 = 51.88; p <0.001). There was a significant increase in the value of the Barthel index after rehabilitation in the patient's home (Z = 19.08; p <0.001).
Findings. The most numerous patients who underwent physical therapy at home were patients with neurological diseases. Among neurological diseases, most patients with cerebral palsy and other paralytic syndromes (G80-G83, MKB10). Maintenance rehabilitation was more prevalent than acute rehabilitation. The largest number of procedures was performed in patients with demyelinating diseases of the nervous system. A significant increase in the value of the Barthel index proved the success of rehabilitation at home.
