Abstract Vezikoureteralni refluks (VUR) je unatražni tok mokraće iz mokraćnog mjehura u ureter, a nerijetko i u bubreg. Najčešća je anomalija mokraćnog sustava i obuhvaća oko 1-2% sve djece. Nastaje zbog insuficijentnosti vezikouretralnog mehanizma koji sprječava povratak mokraće. Simptomi bolesti su često nespecifični, najčešće u smislu infekcija mokraćnog sustava. VUR dijelimo na pasivni i aktivni, a po nalazu mikcijske cistouretrografije dijelimo ga u pet stupnjeva. Zlatni standard za stupnjevanje i dokazivanje VUR-a je mikcijska uretrografija, koju u novije vrijeme zamjenjuje ultrazvučna kontrastna cistografija, koja ima prednost utoliko što nema ionizirajućeg zračenja. Primarni ciljevi u liječenju VUR-a, u djece su prevencija pijelonefritisa te prevencija refluksne nefropatije s mogućom hipertenzijom i zatajenjem bubrega. Liječenje VUR-a može biti konzervativno, kirurško, endoskopsko i laparoskopsko. Danas se najčešće koristi endoskopsko liječenje VUR-a, s vrlo visokom stopom uspjeha.
U Kliniku za dječju kirurgiju KBC-a Split zaprimljeno je dijete koje je operirano zbog vezikouretralnog refluksa. Kako je riječ o jednogodišnjem bolesniku potrebna je stalna prisutnost roditelja. Medicinski tehničar stvara osjećaj sigurnosti, ulijeva povjerenje i sigurnost kod roditelja. Najveći problemi koji su se javili u prijeoperacijskom i poslijeoperacijskom razdoblju zdravstvene skrbi su: strah roditelja u/s s operativnim zahvatom, akutna bol u/s kirurške rane, VR za razvoj infekcije i VR za razvoj hospitalizma u djeteta.
Profesionalan i individualan pristup djetetu i roditeljima te određeni medicinski postupci za koje je zadužen medicinski tehničar pomažu u zdravstvenoj skrbi djeteta, smanjuju strah roditelja, smanjuju hospitalizam u djeteta te ulijevanju snagu i povjerenje što uvekile pomaže boljem oporavku djeteta.
Abstract (english) Vesicoureteral reflux is the abnormal flow of urine from the bladder to the ureters and in some cases to the kidneys. It is the most common abnormality of the urinary tract and affects 1-2% of children. It is caused by an insufficient vesicoureteral mechanism which prevents the urine to flow backwards. The symptoms are often nonspecific, most commonly urinary tract infection. VUR can be primary and secondary. Depending to the voiding cystoureterogram results there are five grades of VUR. Golden standard for establishing diagnosis of VUR was voiding cystouretrogram, but nowadays ultrasound cystography become the most commonly used due to lack of ionizing radiation. Primary goal of treatment for children with VUR is the prevention of pyelonephritis and the prevention of reflux neuropathy with possible hypertension and kidney failure. Treatment of VUR may be conservative, surgical, endoscopic and laparoscopic.
A child was admitted at the Department of pediatric surgery, University Hospital of Split, because of vesicoureteral reflux surgery. Since he was only a one-year-old constant 24 hours a day parent attendance was needed. The nurse develops a safe, trustworthy environment that makes parents feel safe. The biggest problems before and after surgery were fear related to the surgery, pain related to surgical wound, high risk of infection, high risk of developing hospitalism of the child.
A professional and individual approach towards the child and the parents, specific medical procedures were applied by the nurse to help the recovery of the child, reducing fear from the parents and preventing hospitalism of the child, and therefore enabling trust and strength which are a major help in the child’s recovery.
