Abstract
Prevalencija endokrinih bolesti u današnje vrijeme sve je veća pa tako i poremećaji štitne žlijezde. Razlog pojave hipotireoze i hipertireoze su najčešće autoimune bolesti kao što su Hashimotov tireoiditis i Gravesova bolesti. Kako štitnjača utječe na razna tkiva i organe, nastupanjem bolesti remete se metabolički putevi u tijelu. Dolazi do promjena u metabolizmu masti i sveukupnog lipidnog profila te do promjena u metabolizmu i homeostazi glukoze, a postoji i rizik od oboljenja od dijabetesa. Osim navdenog, hipo i hipertireoza uzrokuju hormonske promjene. Dio promjena u metabolizmu određeno je poremećenim lučenjem adiponektina, leptina, rezistina i inzulina. Osobe s dijagnozom hipotireoze najčešće su pretile, s povišenim ukupnim kolesterolom, LDL kolesterolom, smanjenim HDL-om i s povećanom vjerojatnosti za pojavu hipoglikemije. Suprotna slika karakterizira stanje hipertireoze gdje su ukupan i LDL kolesterol smanjeni, HDL može varirati, a osobe mogu biti u stanju hiperglikemije. Liječenje hipotireoze i hipertireoze prvenstveno se provodi primjerenim lijekovima, ali i prehrana može doprinijeti kontroli zdravlja. Određeni spojevi iz hrane mogu smanjiti biodostupnost joda na što treba pripaziti osobito u hipotireozi. Manjak pojedinih minerala kao što su selen, jod, cink, željezo te vitamina pridonosi slabijoj aktivnosti štitnjače. Potrebno je uvesti prehranu koja će osigurati prikladne količine makronutrijenata i mikronutrijenata kako bi se smanjili simptomi bolesti, pospješila pravilna funkcija metabolizma te postigla i održala poželjna tjelesna masa.
Abstract (english)
The prevalence of endocrinological diseases is increasing and so are the thyroid disorders. Autoimmune diseases like Hashimoto's and Graves disease are the most common causes of the hypothyroidism and hyperthyroidism. Because the thyroid gland affects many tissues and organs, some metabolic pathways are modified during the disease. There is a change in fat metabolism and overall lipid profile, as well as changes in metabolism and glucose homeostasis, with higher risk of diabetes mellitus. Changes in hormonal secretion are present as well. Part of the changes include the secretion od adiponectin, leptin, resistin and insulin. Patients diagnosed with hypothyroidism are usually overweight with higher cholesterol levels and higher possibility of hypoglycemia, while those with hyperthyroidism mostly have lower cholesterol levels and episodes of hyperglycaemia. The therapy of thyroid disorders usually includes medications, but a proper nutrition can also contribute to the contol of health. Certain food compounds may reduce the bioavailability of iodine, which should be especially observed in hypothyroidism. The deficiency of certain minerals such as selenium, iodine, zinc and iron as well as some vitamins are associated with weaker thyroid activity. It is necessary to introduce a diet that will provide appropriate amounts of macronutrients and micronutrients to reduce the symptoms of the disease, enhance the proper function of the metabolism and achieve and maintain the desired body weight.
