Abstract
Imunološki eseji su važan alat u istraživanju različitih bolesti ili kliničkoj praksi gdje se koriste u dijagnostičke, ali i prognostičke svrhe. Enzimsko vezani imunoapsorbirajući esej (ELISA) kao pouzdana i validirana singleplex tehnika kojom se mogu određivati pojedini citokini, koristi se kao „zlatni standard“. Međutim, zbog potrebe za boljim razumijevanjem kompleksnih bolesti, potaknut je razvoj eseja koje karakterizira istovremeno određivanje većeg broja različitih citokina unutar istog uzorka, tzv. multiplex eseja. Cilj ovog rada je prikaz i usporedba najčešćih imunoloških tehnika za određivanje većeg broja citokina u istom uzorku kao biomarkera povezanih s multifaktorskim bolestima. Iako multiplex metode imaju veću osjetljivost, točnost, preciznost i nižu granicu detekcije zbog principa na kojem radi sustav za mjerenje citokina, problemi koji se javljaju kao na primjer križna reaktivnost ili nespecifična vezanja, za posljedicu još uvijek imaju njihovu sporadičnu primjenu u kliničkoj praksi zbog nemogućnosti udovoljavanja strogim zahtjevima validacije.
Abstract (english)
Immunoassays are an important tool in the research of different diseases or in clinical practice for diagnostic or prognostic purposes. Enzyme-linked immunosorbent assay (ELISA) is a reliable and validated singleplex technique capable of measuring individual cytokine levels and is used as the gold standard. However, the need for a better understanding of the complex nature of diseases leads to the development of assays characterized by the simultaneous measurement of numerous different cytokines within the same sample, the so called multiplex methods. The goal of this thesis is to present and compare the most common immunological assays for the analysis of proteins (i.e., cytokines) associated with multifactorial diseases. Although such multiplex assays have higher sensitivity, accuracy, precision, and lower limit of detection due to the principle on which the protein measurement system works, at present challenges related to e.g., cross-reactivity or non-specific bonding, are the key reason these assays are intermittently used due to inability to pass rigorous validation procedure in clinical settings.
