Abstract
Žive stanice su najnovija generacija bioloških lijekova tzv. biofarmaceutika, kojima pripadaju T-stanice s kimernim antigenskim receptorima (CAR – engl. chimeric antigen receptor) namijenjene terapiji tumora. CAR T-stanice prepoznaju specifične antigene na površini stanica tumora i uništavaju ih. Prva takva stanična imunoterapija je odobrena 2017. godine, a uključuje također terapiju tumor-infiltrirajućim limfocitima (TIL) i prirodnim stanicama ubojicama (NK, engl. natural killer). Cilj ovog rada je bio opisati mehanizam terapijskog djelovanja CAR T-stanica, njihovu terapijsku primjenu, značaj, prednosti, nedostatke i daljnje mogućnosti razvoja ovog terapijskog pristupa. U svrhu razvoja CAR Tstanica kao lijekova, izolirani limfociti pacijenta se genetički modificiraju ex vivo radi dobivanja rekombinantnih CAR T-stanica koje će imati terapijski učinak. Terapija CAR T-stanicama smatra se revolucionarnom, te se očekuje primjena ovog terapijskog pristupa i za druge karcinome, pa čak i za zarazne i autoimune bolesti.
Abstract (english)
Living cells are the latest generation of biopharmaceuticals, which include T-cells with chimeric antigen receptors (CAR – English chimeric antigen receptor) intended for tumor therapy. CAR T-cells recognize specific antigens on the surface of tumor cells and destroy them. The first cellular immunotherapy was approved in 2017, and it also includes therapy with tumor-infiltrating lymphocytes (TIL) and natural killer cells (NK). The aim of this work was to describe the mechanism of the therapeutic effect of CAR T-cells, their therapeutic application, significance, advantages, disadvantages and further possibilities for the development of this therapeutic approach. In order to develop CAR Tcells as drugs, the patient's isolated lymphocytes are genetically modified ex vivo to obtain recombinant CAR T-cells that will have a therapeutic effect. CAR T-cell therapy is considered revolutionary, and the application of this therapeutic approach is expected for other cancers, and even for infectious and autoimmune diseases.
