Abstract
Vitamin A je vitamin topljiv u mastima i jedan je od esencijalnih nutrijenata potreban za niz metaboličkih procesa u organizmu. U hrani se pojavljuje u dva oblika, kao retinol i ß-karoten. Derivati vitamina A dijele se na sintetske i prirodne oblike koji pripadaju istoj skupini spojeva, retinoidima. Najpoznatiji retinoidi koji se koriste u medicinske svrhe su izotretinoin, alitretinoin, beksaroten, tretinoin i acitretin. Svoje djelovanje ostvaruju vezanjem na dva stanična receptora te djeluju mehanizmom koji još nije u potpunosti razjašnjen. Izotretinoin se uspješno koristi u liječenju upalnih procesa kože, posebice rezistentnih oblika nodularnih akni. Najčešće nuspojave korištenja lijekova čija je aktivna tvar izotretinoin su mukokutane promjene koje prolaze nedugo nakon prestanka terapije, depresija, anksioznost te promjene u raspoloženju i ponašanju, kao i promjena lipidnog statusa te razina jetrenih enzima. Primjena ovog lijeka tijekom trudnoće ozbiljno oštećuje plod te dovodi do velikog broja pobačaja, kao i teških malfomacija fetusa. Također, postoji potencijalna povezanost primjene lijekova na bazi tog derivata vitamina A i upalnih bolesti crijeva.
Abstract (english)
Vitamin A is a fat-soluble vitamin and is one of the essential nutrients required for a number of metabolic processes in the body. It is found in food in two forms, as retinol and ß-carotene. A distinction is made between synthetic and natural forms of vitamin A derivatives, which belong to the same group of compounds, retinoids. The best-known retinoids used for medical purposes are isotretinoin, alitretinoin, bexarotene, tretinoin and acitretin. They exert their effect by binding to two cellular receptors and act by a mechanism that is not yet been fully understood. Isotretinoin is successfully used to treat inflammatory skin processes, particularly resistant forms of nodular acne. The most common side effects associated with the use of drugs whose active ingredient is isotretinoin are mucosal changes that dissapear shortly after discontinuation of therapy, depression, anxiety, mood and behavioral changes as well as changes in lipid status and liver enzyme levels. Taking this drug during pregnancy severely damages the fetus and leads to a large number of miscarriages and severe fetal malformations. There is also a possible link between the use of medicines based on this vitamin A derivative and inflammatory bowel diseases.
