Abstract Posljednjih desetljeća rezultati istraživanja u području ljekarničke djelatnosti ukazali su na intenzivne promjene uloge ljekarnika uz pomak fokusa s proizvoda na aktivnosti usmjerene na pružanje skrbi pacijentu. Cilj ovog istraživanja bio je procijeniti razinu pružanja i odrediti čimbenike koji utječu na pružanje ljekarničke skrbi u javnim ljekarnama na području Hrvatske, te usporediti dobivene rezultate s istraživanjima iz drugih europskih zemalja. Presječno istraživanje se provodilo od prosinca 2017. do veljače 2018. godine, a uključivalo je magistre farmacije u ljekarnama širom Republike Hrvatske koji su imali status mentora-ljekarnika. Metoda prikupljanja podataka bila je modificirana verzija BPCS (engl. Bihevioral pharmaceutical care scale, BPCS) upitnika. U istraživanju je sudjelovao 181 ispitanik od ukupno pozvanih 330, čineći stopu odaziva 54,8%, pri čemu je bilo 163 žene i 18 muškaraca. Srednja vrijednost radnog staža iznosila je 15,15 ± 9,06 godina, a kretala se u rasponu od 1 - 39 godina. Zbrojem bodova upitnika prema BPCS skali dobiven je ukupan broj bodova koji je predstavljao razinu pružanja ljekarničke skrbi. Mogući raspon kretao se od 15 do 160, a srednja vrijednost BPCS bodova iznosila je 80,66 ± 22,36, što ne odstupa od rezultata dobivenih u drugim europskim zemljama, ali još uvijek predstavlja ograničenu ljekarničku skrb. Ovo istraživanje prvi puta je na uzorku hrvatske populacije istražilo pružanje ljekarničke skrbi u ljekarnama korištenjem modificirane verzije BPCS upitnika. Pokazalo se da magistri farmacije najviše sudjeluju u aktivnostima izravno usmjerenim na pacijente, dok su rjeđe pružali ljekarničku skrb koja uključuje komunikaciju i suradnju s drugim zdravstvenim radnicima. Ispitanici koji su imali višu razinu pružanja ljekarničke skrbi, provodili su više aktivnosti u ljekarni- češće su provjeravali interakcije lijekova, kontraindikacije i kliničke podatke bolesnika. Čimbenici koji još uvijek ograničavaju ljekarničku skrb su nedostatak ili nekorištenje prostora za konzultacije i nedostupnost kliničkim podacima pacijenata te računalnim programima za provjeru interakcija i drugih značajnih informacija o lijekovima.
Abstract (english) Over the past decades, evidence indicates intense changes in the field of pharmacy practice and the shift of pharmacists’ role focus from product to patient care activities. The aim of this study was to assess the current degree of provision of pharmaceutical care by community pharmacists in Croatia, to determine the factors that influence the provision of pharmaceutical care and to compare the results with studies from other European countries. The cross-sectional study was conducted from December 2017 to February 2018 and included community pharmacists throughout Croatia who had a status of students’ mentors at the Faculty of Pharmacy and Biochemistry, University of Zagreb. A questionnaire comprising of demographics, pharmacy services and layout and modified version of Behavioural pharmaceutical care scale (BPCS) was used for data collection. Study included 181 respondents from the total of 330 pharmacists who were invited in the study, with the response rate of 54,8%. 163 women and 18 men participated in the study. The average duration of employment was 15,15±9,06 years and ranged from one to 39 years. The total number of BPCS scores was calculated and represented the degree of provision of pharmaceutical care. Total BPCS scores achieved were 80,66 ± 22,36 (possible score range being 15 to 160), which does not deviate from the results obtained in other European countries, but still represents limited pharmaceutical care. This is the first study that examined the provision of pharmaceutical care in community pharmacies, using a modified version of BPCS questionnaire in a sample of Croatian population. The highest scores were noted for direct patient care activities, while lowest scores were achieved in referral and consultation activity dimension. Respondents, who scored higher on overall pharmaceutical care provision, were more likely to check drug interactions, contraindications and patients’ clinical data and altogether carried out more activities in pharmacy. Factors that still limit the provision of pharmaceutical care include the non-availability of patient consultation area or database for drug interactions analysis as well as non-availability of patients’ clinical data.
