Main characteristics of chronic obstructive pulmonary disease, obstruction of the airways and tissue damage, are caused by chronic and progressive inflammation. Exposure to tobacco smoke or other foreign particles causes oxidative stress and promotes production of cytokines and infiltration of the inflammatory cells, which further cause tissue damage. Two molecules that might be involved in inflammatory processes connected with COPD progression are extracellular Hsp70 and ATP. The source of those molecules in the extracellular matrix are damaged cells, and epithelial and immune cells which release them during inflammation. Both molecules interact with the immune system and are markers of tissue damage. Despite COPD's large prevalence, diagnostic methods are somewhat inadequate, and a large number of patients are undiagnosed or are not receiving appropriate treatment. The goal of this research was to assess concentrations of eHsp70 and eATP in patients diagnosed with COPD and to compare them with a control group, regarding their smoking status and GOLD classifications. The concentrations of both parameters, eHsp70 and eATP, were increased in the patient group when compared to the control group, increased with the severity of the disease, and differed significantly between each GOLD group. The concentrations of eHsp70 and eATP were increased in patients regardless of their smoking status when compared to the healthy smokers and healthy nonsmokers. However, in the group of healthy smokers the concentrations of both parameters were similar to groups GOLD 2 and GOLD A. That could indicate some initial inflammatory changes and a possibility of later COPD development in genetically or otherwise susceptible individuals. In conclusion, the concentrations of extracellular Hsp70 and ATP were increased in patients diagnosed with COPD, and the concentration increase shows correlation with the progression of the disease.
