Abstract
Atopijski dermatitis (AD) predstavlja jednu od najčešćih kroničnih upalnih bolesti kože. Uobičajena terapija blagih i srednje teških oblika AD-a uglavnom se zasniva na topikalnim kortikosteroidima i inhibitorima kalcineurina čija učestala primjena sa sobom povlači i brojne lokalne nuspojave, što se može izbjeći uklapanjem lijekova u odgovarajuće nanosustave. Upravo zato se unazad dvadesetak godina nanosustavi intenzivno proučavaju kao potencijalno bolji načini formulacije lijekova za topikalnu primjenu. Njihova superiornost proizlazi iz činjenice da znatno učinkovitije dostavljaju lijekove na njihovo ciljno mjesto djelovanja, čime se smanjuje količina lijeka u formulaciji potrebna za ispoljavanje terapijskog učinka, a samim time i vjerojatnost nuspojava. Uslijed specifičnog načina uklapanja lijekova unutar nanosustava, moguće je prilagoditi oslobađanje lijekova te zaštititi nestabilne lijekove od razgradnje, poboljšati njihove karakteristike i postići bolju penetraciju i permeaciju u kožu. Ovaj rad donosi pregled istraživanja primjene nanosustava u liječenju AD-a. Ukratko je opisana specifičnost građe kože u pacijenata s AD-om te mehanizmi penetracije lijekova i nanosustava kako bi se dodatno pojasnio kontekst primjene nanosustava u topikalnoj terapiji AD-a.
Abstract (english)
Atopic dermatitis (AD) is one of the most common chronic skin diseases. Conventional therapy for mild to moderate forms of AD is mainly based on topical corticosteroids and calcineurin inhibitors whose frequent use entails a number of local side effects, which can be overcome by their encapsulation in various nanocarriers (nanosystems). Therefore, nanosystems have been intensively studied for the past twenty years as a potentially better way of formulating drugs for topical application. Their superiority stems from the fact that they can deliver drugs much more efficiently to their target site of action, thus reducing the amount of drug in the formulation required to exert desired therapeutic effect, along with the reduced incidence of side effects. Moreover, it is possible to modify drug release, protect unstable drugs from degradation and improve their penetration in the skin. This study provides an overview of drug delivery nanosystems for the topical AD treatment. The structure of the skin is briefly described, as well as its differences in patients with AD, along with the interactions of the nanosystems and skin to clarify the effect of nanosystems in topical AD therapy.
