Abstract
Dijabetes je skupina metaboličkih poremećaja koje karakterizira kronično povišena razina glukoze u krvi. Jedna od brojnih komplikacija dijabetesa je dijabetička polineuropatija koja može značajno utjecati na kvalitetu života pacijenta. Pretilost danas predstavlja sve veći javnozdravstveni problem koji se povezuje s povećanim rizikom za pojavu kardiovaskularnih bolesti, dijabetesa i niza karcinoma. S obzirom na javnozdravstveni značaj pretilosti i dijabetičke polineuropatije, postoji veliki interes za razvojem novih adjuvantnih terapija koje bi mogle doprinjeti prevenciji i liječenju navedenih stanja. Cilj ovog istraživanja bio je pregledom literature utvrditi značaj α-lipoične kiseline u terapiji dijabetičke polineuropatije i pretilosti. α-lipoična kiselina je nutraceutik s izraženim antioksidativnim i protuupalnim učincima; značajnim učincima na regulaciju apetita te sposobnošću poboljšanja transporta glukoze i inzulinske osjetljivosti. Rezultati kliničkih studija pokazuju da ona pozitivno utječe na određene parametre kod pacijenata s dijabetičkom polineuropatijom, prvenstveno na provodljivost motoričkih i senzornih živaca. Također, čini se da α-lipoična kiselina može pozitivno utjecati na neke parametre pretilosti, iako su razine dokaza za navedenu indikaciju vrlo niske. Kako bi se zaista utvrdio terapijski značaj α-lipoične kiseline u terapiji dijabetičke polineuropatije i pretilosti, potrebne su dodatne kliničke studije koje će primijenjivati bolje metodološke prakse.
Abstract (english)
Diabetes is a group of metabolic disorders characterised by a chronic elevation of blood sugar levels. One of its many complications is diabetic polyneuropathy, which is known to have a drastic effect on diabetic patients’ quality of life. Obesity significantly increases the risk of cardiovascular events, diabetes and different types of cancer and therefore represents a rapidly growing public health issue. Considering the public health significance of obesity and diabetic polyneuropathy there is a major interest in development of new adjuvant therapies for their prevention and treatment. The aim of this study was to review the literature to determine the importance of α-lipoic acid in the treatment of diabetic polyneuropathy and obesity. α-lipoic acid is nutraceutical with antioxidant and anti-inflammatory properties, impact on food intake, and beneficial effects on glucose transport and insulin sensitivity. Results of clinical studies show that it can exert significant impacts on specific parameters of diabetic polyneuropathy, primarily on the nerve conduction velocity (NCV). Furthermore, it has been shown that α-lipoic acid can positively influence certain obesity markers, although the level of evidence for those effects is rather low. In order to clearly establish the therapeutic significance of α-lipoic acid supplementation in treating diabetic polyneuropathy and obesity, it is necessary to conduct additional high quality clinical studies.
