Research objectives: The aim of this paper is to determine the nature, frequency and potential severity of errors in prescribing psychopharmaceuticals and other medication errors in psychiatry. The focus is also on the role of the pharmacist in a multidisciplinary hospital team and their importance in detection and prevention of such errors in the future. Material and methods: For the purpose of researching for this paper, a systematic review was made of relevant books and psychiatric manuals, scientific and professional literature, publications of professional institutions, bibliographic databases (Pubmed) and web pages with drug databases (HALMED). Results: Medication error has been found to be a frequent and crucial problem that affects up to 4% inpatients, and in the last few years their number has increased. Medication errors contribute to increased morbidity and mortality, as well as prolong the hospitalization time of patients. As much as 25% of all civil procedures in medical practice are initiated because of medication errors. Conclusion: In order to prevent medication errors in the scope of medical errors, it is recommended to take a detailed medical history of every patient, approach patient individually, make sure the entire multidisciplinary team is involved in decision-making regarding drug prescription, and implement a monitoring system to increase the safe use of medication as well as a reporting system for medical errors that have already occured.
