| Abstract | Cilj istraživanja
Cilj ovog specijalističkog rada bio pregledno prikazati, analizirati i raspraviti pojavu nitrozaminskih onečišćenja u lijekovima te posljedične regulatorne smjernice Europske agencije za lijekove i Europske komisije.
Materijal i Metode
Literatura je pretraživana prema temi istraživanja, predmetu istraživanja, autorima i časopisu, od općih prema specijaliziranim člancima pri čemu su odabrani članci i smjernice relevantne za problematiku specijalističkog rada. Pri pretraživanju literature i prikupljanju relevantnih podataka za izradu specijalističkog rada korištene su on-line baze podataka (PubMed, ScienceDirect, Elsevier, mrežna stranica Agencije za lijekove i medicinske proizvode i mrežne stranice Europske agencije za lijekove) te relevantne smjernice vezane za nitrozaminska onečišćenja. U ovom dijelu rada predstavljen je sustavni pregled saznanja o nitrozaminskim onečišćenjima u lijekovima.
Rezultati
Nitrozamini su odavno poznati kao kancerogene tvari prisutne u hrani, kozmetici te duhanskom dimu, no otkriće nitrozamina u valsartanu 2018. godine dovelo je do detaljnijih analiza koje su ukazale na raširenu prisutnost nitrozamina i u lijekovima. Pronalazak nitrozaminskih onečišćenja u valsartanu potaknuo je niz regulatornih akcija koji su isprva bili ograničeni na skupinu sartana s tetrazolnim prstenom, ali su se vrlo brzo proširili i na druge lijekove. Isprva su se provjeravale djelatne tvari, ali ubrzo se pokazalo da nitrozamni mogu biti prisutni i u gotovom lijeku te da postoji čitav niz potencijalnih uzroka prisutnosti nitrozamina. S obzirom na kancerogeni potencijal nitrozamina takvim otkrićem javila se zabrinutost od strane regulatornih agencija diljem svijeta. Europska agencija za lijekove provela je opsežnu istragu uzroka prisutnosti nitrozamina te metoda prevencije nastanka, detekcije i kontrole. U periodu od 2018. do 2023. uspostavljene su brojne smjernice i sigurnosne granične vrijednosti za nitrozamine u lijekovima. Granične vrijednosti su se mijenjale kako su se saznanja i istraživanja razvijali. Taj posao nije završen, i dalje se svako malo otkrije neki novi nitrozamin specifičan za određenu djelatnu tvar ili lijek, a regulatorne agencije kao i farmaceutska industrija morat će biti u toku s novim saznanjima te prilagođavati svoje smjernice i procese.
Zaključak
Kao regulatorni rezultat istrage nitrozamina nositelji odobrenja svih lijekova (kemijskih i bioloških) na tržištu Europske unije dužni su provesti procjenu rizika prisutnosti nitrozamina. Ako se potvrdnim ispitivanjima dokaže prisutnost nitrozamina u razini višoj od propisanih dnevnih limita, nositelji odobrenja dužni su putem izmjena/e odobrenja za stavljanje lijeka u promet prilagoditi proizvodne procese kako bi se prisutnost nitrozamina uklonila ili smanjila na minimum. Budući da kontaminacija nitrozaminima utječe na pacijente diljem svijeta, regulatorna tijela Europske unije, poput EMA-e, surađuju s međunarodnim partnerima i nastavit će poduzimati potrebne mjere kako bi osigurali da se proizvođači i nositelji odobrenja za stavljanje u promet pridržavaju preporuka u vezi s nitrozaminima i drugim onečišćenjima, istovremeno osiguravajući da pacijenti imaju pristup sigurnim i učinkovitim lijekovima. |
| Abstract (english) | Objectives
The aim of the this expert thesis is to present, analyze and discuss the occurrence of nitrosamine contaminants in medicines and related regulatory guidelines of the European Medicines Agency and the European Commission.
Material and Methods
The literature was searched according to the research topic, research subject, authors and journal, from general to specialized articles, with selected articles and guidelines relevant to the issue of specialist work. Online databases (PubMed, ScienceDirect, the website of the Agency for Medicines and Medical Products and the website of the EMA) and relevant guidelines related to nitrosamine impurities were used when searching the literature and collecting relevant data for the preparation of this paper.
Results
Nitrosamines have long been known as carcinogenic substances present in food, cosmetics and tobacco smoke, but the discovery of nitrosamines in valsartan in 2018 led to more detailed analyzes that indicated the widespread presence of nitrosamines in medicinal products as well. The discovery of nitrosamine impurities in valsartan triggered a series of regulatory actions that were initially limited to the group of sartans with a tetrazole ring, but very quickly spread to other drugs as well. At first, the active substances were checked, but it soon became clear that nitrosamines can also be present in the finished medicine and that there is a whole series of potential causes of the presence of nitrosamines. Given the carcinogenic potential of nitrosamines, this discovery has raised concerns from regulatory agencies around the world. The European Medicines Agency has conducted an extensive investigation into the causes of the presence of nitrosamines and methods of prevention, detection and control. In the period from 2018 to 2023, numerous guidelines and safety limits for nitrosamines in medicines were established. These boundaries changed as knowledge and research developed. This work is not finished, every now and then a new nitrosamine specific for a certain active substance or medicine is discovered, and regulatory agencies as well as the pharmaceutical industry will have to keep up with new knowledge and adjust their guidelines and processes.
Conclusion
As a regulatory result of the investigation of nitrosamines, the holders of the approval of all drugs on the European Union market, both chemical and biological, are obliged to carry out a risk assessment of the presence of nitrosamines. If the confirmatory tests prove the presence of nitrosamines at a level higher than the prescribed daily limits, the authorization holders are obliged to adjust the production processes in order to remove or reduce the presence of nitrosamines to a minimum by means of variations to the marketing authorization. As nitrosamine contamination affects patients worldwide, EU regulatory bodies such as the EMA are working with international partners and will continue to take the necessary measures to ensure that manufacturers and marketing authorization holders comply with nitrosamine recommendations and other impurities, while ensuring that patients have access to safe and effective medicines. |
