Abstract
Oralna hitna kontracepcija predstavlja veliki napredak za društvo, osobito žene u pogledu planiranja obitelji. Danas su u Hrvatskoj registrirana dva preparata za oralnu hitnu kontracepciju: preparat s levonorgestrelom (sintetski gestagen) i ne-hormonalni preparat s ulipristal acetatom (selektivni modulator progesteronskih receptora). Oba su u bezreceptnom režimu izdavanja jer je procjenjeno da je omjer koristi i rizika pozitivan. Prema pregnancy rate, oba preparata imaju podjednaku djelotvornost unutar 72 sata, dok je u perioiodu između 72 i 120h s obzirom na djelotvornost u prednosti ulipristal acetat. Logika nalaže da je prebacivanje hitne oralne kontracepcije iz receptnog u bezreceptni režim donijelo uštede zdravstvenom sustavu. Veća dostupnost je omogućila pravovremeni pristup pacijentice lijeku (osobito ženama koje nemaju ginekologa te u vrijeme blagdana i vikenda) te je povećan broj sprječenih neželjenih trudnoća. Oralni hitni kontraceptivi imaju pozitivan omjer cijene i učinka neovisno o tome nabavi li se kad se pojavi potreba ili preventivno. Učinak je vidljiv u medicinskim i socijalnim uštedama uslijed izbjegavanja neželjene trudnoće, a nije zanemariv ni pozitivan psihološki učinak.
Abstract (english)
Oral emergency contraception is a major plus for society, especially women in terms of family planning. Today in Croatia there are two registered products for oral emergency contraception: preparation with levonorgestrel (a synthetic progestogen) and non-hormonal preparation with ulipristal acetate (selective progesterone receptor modulator). Both are in OTC mode of issue because it is estimated that the benefit-risk ratio is positive. According to pregnancy rate, both products have similar effectiveness within 72 hours, between 72 and 120h ulipristal acetate has the advantage. Logic dictates that the transfer of emergency oral contraception from prescription to OTC regime brought savings to the health care system. The increased availability allowed timely access to the drug (especially to women who do not have a gynecologist and during the holidays and weekends) and decreased the number of unwanted pregnancies. Emergency oral contraception has a positive cost-effectiveness ratio regardless if it is obtained when the need arises or as a prevention. The effect is visible in the medical and social savings due to avoidance of unwanted pregnancies and there is a significant positive psychological effect.
