Abstract
Bol nastala kao posljedica ozljede leđne moždine obuhvaća nociceptivnu, neuropatsku, visceralnu te bol središnjeg porijekla. Usprkos brojnim suvremenim metodama liječenja, kod velikog broja ovakvih pacijenata, neadekvatno i nepravovremeno liječenje bolova rezultira kroničnim bolnim sindromom, pri čemu bol preuzima dominaciju nad njihovim životom. Cilj ovog rada je istražiti i opisati načine liječenja boli te ukazati na probleme i izazove kod osoba s ozljedom leđne moždine. U liječenju se najbolje pokazala kombinacija farmakoterapije s nefarmakološkim postupcima. Kao temelj racionalne primjene analgetika preporuča se trostupanjska ljestvica Svjetske zdravstvene organizacije. Kod odluke o odabiru analgetika važno je procijeniti o kakvoj je vrsti boli riječ, te uzeti u obzir čimbenike koji mogu modificirati odgovor na lijek: npr. oštećenje bubrežne ili jetrene funkcije, dob, postojanje kroničnih bolesti, polimorfizam gena koji sudjeluju u metabolizmu, interakcije lijekova. Treba također prilagoditi lijek pacijentu, njegovim životnim navikama i općem stanju.
Abstract (english)
Pain caused by injury of spinal cord includes nociceptive, neuropathic, visceral and central pain. Despite many modern methods of treatment, inadequate and untimely treatment of pain results in cronic pain syndroms in a large group of patients where pain takes over their life. The purpose of this graduation thesis is to research and explain treatments of pain and indicate problems and challenges with patients that suffer from spinal cord injury. The best treatment of pain is the combination of pharmacotherapy and nonpharmacological procedures. Three-stage scale by world health organization is recommended as the basis of rational usage of analgetics. When deciding which analgetic to use, it is important to estimate type of pain and take in consideration all factors which can modify response to pain such as damaged function of kidney or liver, age, chronic disease, gene polimorphism which has an important role in metabolism, medicine interaction. As well the medicine should be adapted to patients life habits and general condition.
