Abstract
Cilj rada je utvrđivanje aktivacije komplementa u transplantiranoj jetri. Ispitano je 113 ortotopnih transplantacija jetre, kod 107 pacijenata, u razdoblju od siječnja 2008. do prosinca 2009. godine. Iz postojećih podataka, te provođenjem novih testiranja na pohranjenim uzorcima seruma i tkiva, analizirana je prisutnost odbacivanja u presatku, postojanje antidonorskih HLA protutijela Luminex testom i CDC testom, te je imunohistokemijski utvrđivana izraženost C4d komponente (biljega aktivacije komplementa u presatku). Rezultati potvrđuju čestu aktivaciju komplementa u presatku, prisutnu u različitim oštećenjima, upućujući na različite puteve aktivacije kaskade komplementa. U "nultim" biopsijama češća je centrilobularna, a u biopsijama s potvrđenim odbacivanjem portalna lokalizacija C4d. Potvrđena je pozitivna korelacija odbacivanja i izraženosti C4d komponente komplementa. Identificirana je manja skupina ispitanika kojima su potvrđena specifična HLA protutijela u serumu, a u kojoj su češće bile i epizode akutnog odbacivanja i kompliciraniji poslijetransplantacijski tijek, što upućuje na značaj protutijela i aktivacije komplementa u oštećenju presatka.
Abstract (english)
Aim of the study is to determine complement activation in transplanted liver. We studied 113 orthotopic liver transplantations in 107 patients, from January 2008 until December 2009. We ana-lyzed graft rejection using existing data and further testing of stored serum and tissue samples, tested for presence of HLA-antibodies by Luminex or CDC tests and determined the C4d com-plement component (marker of complement activation). Our results confirmed frequent comple-ment activation in the graft, present in different injuries, suggesting various complement activation paths. Centrilobular localization of C4d is more common in «zero» biopsies, while portal localiza-tion is more frequent in biopsies with confirmed rejection. We showed a positive correlation be-tween rejection and C4d expression. We identified a small group of subjects with positive serum HLA-antibodies and more frequent episodes of rejections as well as a more complicated postoper-ative recovery, suggesting a significant role of antibodies and compliment activation in graft inju-ry.
