Abstract
Hemokromatoza je nasljedna bolest koja se manifestira prekomjernim nakupljanjem željeza u tkivima. Može biti nasljedna i stečena ali njen najčešći uzrok su mutacije u genu HFE koje onemogućuju sintezu hepcidina, proteina odgovornog za regulaciju iskorištavanja željeza. Najčešće mutacije koje uzrokuju hemokromatozu su mutacije C282Y, H63D i S65C. Kako bi se neka mutacija mogla korisitit u dijagnostičke svrhe, kao marker pojedinih stanja, potrebno je verificirati protokol kojim se ona dokazuje.... More Verifikacijski protokol za uvođenje metode za detekciju mutacija povezanih s pojavom hemokromatoze podrazumijevao je procjenu usporedivosti, ponovljivosti i točnosti metode. Ispitivanje usporedivosti učinjeno je pomoću 26 uzoraka za mutaciju C282Y, 25 za mutaciju H63D i 17 za mutaciju S65C. Uspoređeni su rezultati dobiveni analizom tih uzoraka i rezultati koji za te uzorke postoje u laboratoriju KBC-a Zagreb. Preciznost istraživane metode utvrđena je korištenjem tri uzorka. Svaka mutacija opetovano je određivana u istom uzorku te su ti podaci međusobno uspoređeni. Parametar točnosti prikazan je kao sličnost rezultata četiri ispitivana uzorka i uzoraka referentne vrijednosti vanjske ustanove Reference Institute for Bioanalytics. Svaki od istraživanih parametara pokazao je 100% podudaranje, međutim zbog mnogih ponavljanja PCR-analiza zbog neuspjelog umnažanja pojedinih uzoraka u prvotnim analizama PCR, istraživana metoda za genotipizaciju mutacija H63D, S65C i C282Y nije uvedena kao dijagnostička metoda u laboratorij bolnice KB Sveti Duh. Abstract (english)
Hemochromatosis is an inherited disease characterized by excessive accumulation of iron in tissues. It can be hereditary or acquired, but its most common causes are mutations in the HFE gene, which impair the synthesis of hepcidin responsible for regulating iron utilization. The most common mutations causing hemochromatosis are C282Y, H63D, and S65C mutations. In order for a mutation to be used as a biomarker for diagnostic purposes it is necessary to verify the protocol by which it is... More detected. The verification protocol for introducing new method for detection of mutations associated with the onset of hemochromatosis involved assessing the comparability, repeatability, and accuracy of the method. Comparability testing was done using 26 samples for the C282Y mutation, 25 samples for the H63D mutation, and 17 samples for the S65C mutation testing. Results gained using the new method were compared with already known results provided by the Clinical Hospital Center Zagreb. Precision testing used three samples. Similarity of results was assessed by repeated measurements of the same sample under the same conditions, and the accuracy parameter showed the similarity of the result of four tested samples and reference samples from the External Quality Control Reference Institute for Bioanalytics. Each of the parameters matched in 100 %. However, due to many repetitions of PCR analysis because of failed amplification of some samples in the initial PCR analyses, the method for genotyping mutations H63D, S65C, and C282Y was not introduced as a diagnostic method at the laboratory in Clinical Hospital Sveti Duh.
