Abstract
Tumorske stanice same ili stanice domaćina proizvode određene tvari kao odgovor na tumor. Ukoliko se njihova razina može izmjeriti u tjelesnim tekućinama ili tkivu pacijenta iako mogu služiti za razlikovanje tumorskog i zdravog tkiva, nazivaju se tumorski markeri. Mnoga istraživanja se provode za otkrivanje novih i razumijevanje već otkrivenih markera za tumore mozga. Gliom je tip tumora mozga koji najčešće nastaje iz glija stanica. Gliomi se dijele u razrede ovisno o malignosti. Najviši, IV razred glioma čine glioblastomi. Glioblastom je najčešći primarni tumor mozga kod odraslih pacijenata s prosječnim preživljenjem14 mjeseci nakon dijagnoze. Potencijalni geni i enzimi koji bi mogli služiti kao tumorski markeri glioblastoma su O-6-metilgvanin-DNA-metiltransferaze (MGMT), geni izocitrat dehidrogenaze 1 i 2 (IDH1, IDH2), p53, receptor za epidermalni faktor rasta (EGFR), receptor trombocitnog faktora rasta (PDGFR), homolog fosfataza i tenzina (PTEN), fosfoinozitid-3-kinaza (PI3K) i kodelecija1p/19q. Dosadašnjim istraživanjima potvrđena je važnost MGMT i IDH1/2 gena u tumorigenezi glioblastoma, dok se uloga ostalih navedenih markera još ne može pouzdano dokazati. Postojeći markeri većinom ne pokazuju dovoljnu specifičnost da bi se mogli koristiti u ranoj dijagnostici, no ipak pružaju važne informacije koje se mogu koristiti uz ostale dijagnostičke metode.
Abstract (english)
Tumor cells or host cells produce certain substances in response to the tumor. Such substances, if measurable in body fluids or tissues of a patient that can serve in distinguishing tumor and healthytissue are called tumor markers. Many studies are trying to develop new and to understand already known markers for brain tumors. Glioma is a type of brain tumor that usually arises from glial cells. Gliomas are divided into classes depending on the agressivnes. The highest, grade IV gliomas are glioblastomas. Glioblastoma is the most common primary brain tumor in adults patients with a average survival 14 months after diagnosis. Potential genes and enzymes that could serve as a glioblastoma tumor markers are O-6-methylguanine-DNA methyltransferase (MGMT), isocitratedehydrogenase genes 1 and 2 (IDH1, IDH2), p53, epidermal growth factor receptor (EGFR), platelet derived growth factor receptor (PDGFR), phosphatase and tensin homologue (PTEN), phosphoinositide-3-kinase (PI3K) and 1p/19q codeletion. Previous studies confirmed the importance of MGMT and IDH1/2 genes in glioblastoma tumorigenesis, while the role of other above mentioned markers still can not be confirmed. Existing markers mostly do not show sufficient specificity that could be used in early diagnosis, but still provide important information that can be used with other diagnostic methods.
