Abstract
Tumori su nenormalna nakupina stanica koja nastaje proliferacijom stanice te nakupljanjem pogodnih genomskih promjena. Takva stanica prestaje surađivati sa stanicama u održavanju homeostaze. Tumorska populacija je heterogena te je sastav stanica varijabilan koja u ovisnosti o mikrookolišu. Stanice sa pogodnim fenotipom preživljavaju te stvaraju subklonske populacije, dok neprilagođene stanice te njihove subklonske populacije izumiru. Također, tumorske stanice mogu imati različite fenotipe koji im omogućavaju preživljavanje i proliferaciju u konstantno promjenjivim uvjetima. Sama heterogenost tumora posljedica je izdašnih genomskih promjena; mutacije gena i epigenetske promjene dovode do spore evolucije koju, u nekim tipovima tumora, prekidaju katastrofični događaji koji dovode do promjene kariotipa svake pojedine stanice, što u veoma kratkome roku još i više povećava heterogenost populacije, tj. ubrzava evoluciju. Tumorske stanice su u kooperaciji sa ostalim tumorskim stanicama, ali i u interakciji sa normalnim stanicama. Tumori mijenjaju svoj mikrookoliš na način da utječu na normalne stanice produktima metabolizma ili putem signalnih molekula. Mijenjanje okoliša uključuje promjene normalnih stanica ali i degradaciju vanstaničnog matriksa. Progresija tumora završava smrću organizma, što je posljedica metastaza. Metastaze su kompleksan proces pri kojemu stanice napuštaju tumor te koloniziraju udaljena tkiva. Stanice sposobne za metastaze su preadaptirane u primarnom tumoru te imaju posebne prilagodbe koje im omogućuju preživljavanje i proliferaciju u udaljenim tkivima.
Abstract (english)
Tumors are abnormal clusters of cells that arise when a cell acquires advantageous set of mutations that cause abnormal proliferation and evading cell death. Tumor populations are heterogeneous clusters with variable cell composition according to changes in microenvironment. Cells with suitable phenotype survive, divide and give rise to subclonal populations, while cells that do not have suitable phenotype go extinct. Furthermore, some tumor cells show phenotypes that allow them to survive and proliferate in constantly changing microenvironment. Heterogeneity is a result of various genomic changes. Gene mutations and epigenetic changes lead to gradual evolution, while chromosomal changes lead to punctuated evolution. Also, tumor cells are in constant cooperation with other tumor and normal cells. Tumor cells change their environment, including other cells and extracellular matrix, with products of metabolism or with various signaling molecules. Tumor progression leads to metastasis that eventually kill the host. Metastasis are a complex process by which tumor cells colonize distant tissues. Cells capable of colonizing distant tissues are pre-adapted to microenvironment in distant tissue.
