Abstract
Karcinom pluća je među najučestalijim malignim bolestima u svijetu s oko 1,7 milijuna novih smrtnih slučajeva u 2018. godini. Osim dima cigarete, kao najznačajnijeg faktora, karcinom pluća se razvija jednim dijelom i kod nepušača te se kao uzrok istražuju i infektivni agensi. Virusi se već dugo vremena opisuju kao etiološki faktor u tumorigenezi, a onkološki potencijal citomegalovirusa (CMV, od eng. –cytomegalovirus), ljudskog papiloma virusa (HPV, od eng. – human papillomavirus) i virusa Epstein-Barr (EBV, od eng. – Epstein-Barr virus) je već istraživan u različitim karcinomima. Ulazak virusa u stanicu je određen interakcijom između virusa i receptora na staničnoj površini domaćina, a kao jedan od značajnih receptora opisan je receptor epidermnog faktora rasta (EGFR, eng.– epidermal growth factor receptor). Oko 12% adenokarcinoma pluća ima jednu od mutacija EGFR-a. S obzirom na povezanost mutacija EGFR-a i virusne etiologije malignih tumora epitelnih stanica cilj ovog istraživanja je bio utvrditi učestalost infekcije CMV-om, HPV-om i EBV-om kod pacijenata s karcinomom pluća koji imaju mutaciju EGFR-a. Metodom lančane reakcije polimerazom detektirana je DNA virusa CMV-a, EBV-a i HPV-a u stanicama adenokarcinoma s mutacijom EGFR-a i u stanicama adenokarcinoma kod kojih nije dokazana mutacija EGFR-a. Rezultati su pokazali statistički značajnu razliku (p<0,001) u prisutnosti DNA sva tri virusa u stanicama adenokarcinoma s mutacijom EGFR-a u odnosu na stanice adenokarcinoma bez mutacije EGFR-a.
Abstract (english)
Lung cancer is one of the most common malignant disease in the world with estimated 1,7 million new death cases in 2018. Even though cigarette smoking is considered by far the single most important risk factor in lung carcinogenesis, smaller percentage of non-smokers also develop lung cancer and one of the other risk factors considered are infectious agents. Oncogenic viruses have been proposed as etiological factor in carcinogenesis with the focus on oncological potential of cytomegalovirus (CMV), human papillomavirus (HPV) and Epstein-Barr virus (EBV) in different carcinomas. Virus entry in the host cell usually starts with virus interaction with host cell surface receptors and epidermal growth factor receptor (EGFR) has been described as one of the most important. About 12% of lung adenocarcinomas carries one of the known EGFR mutations. Considering observed relationship between EGFR mutations and virus etiology of epithelial cell malignant tumors, the aim of this research was to establish frequency of infections with CMV, HPV and EBV in lung cancer patients who have one of EGFR mutations. Polymerase chain reaction was used to detect CMV, HPV and EBV DNA in adenocarcinoma cells with EGFR mutation, as well as in adenocarcinoma cells without EGFR mutation. Results have shown statistically significant difference (p<0.001) in presence of viruses in adenocarcinoma cells with EGFR mutation in comparison with adenocarcinoma cells without EGFR mutation.
